THE DEVELOPMENT OF PRESCRIPTION SCREENING SYSTEM IN OUTPATIENTS PHARMACY SERVICES NONGKHAI HOSPITAL AT NONGKHAI PROVINCE

Abstract

This action research aimed to develop a prescription screening system. The three steps of action research followed were: basic information and obstacle analysis, development of a prescription screening system and implementation, and prescription errors assessment. Tools used were guidelines for prescription screening, a prescription screening form, a medication error record form, and risk reporting in the HRMS program. Data were collected during two time periods: between October 2023 and March 2024, and between July 2024 and December 2024. Data were analyzed using descriptive statistics.

The results found that after the prescription screening system was developed, the incidence of detected prescription errors increased from 1.826 per 1,000 prescriptions to 2.815 per 1,000 prescriptions due to screening by pharmacists. The severity of prescription errors was classified as type B only. Pre-dispensing errors in the prescription order entry process were reduced from 0.574 per 1,000 prescriptions to 0.185 per 1,000 prescriptions. Physicians accepted the pharmacists' consultations 430 times, representing a 99.31 percent acceptance rate.

The development of the prescription screening system and screening by pharmacists effectively prevented medication errors from reaching patients.
