ปัญหาการดูแลสุขภาพและการเข้าถึงบริการสุขภาพของเด็กที่มีความพิการบนใบหน้าและกะโหลกศีรษะแต่กำเนิด: การรับรู้ของผู้ดูแล

วัตถุประสงค์: เพื่อศึกษาปัญหาการดูแลสุขภาพและการเข้าถึงบริการสุขภาพของเด็กที่มีความพิการบนใบหน้าและกะโหลกศีรษะแต่กำเนิดที่ไม่ใช่ปากแหว่งเพดานโหว่ตามการรับรู้ของผู้ดูแล
การออกแบบวิจัย: การวิจัยเชิงพรรณนา
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ดูแลหลักของเด็กที่มารับการตรวจรักษาที่โรงพยาบาลระดับตติยภูมิขั้นสูงแห่งหนึ่งในกรุงเทพมหานคร โดยการตอบแบบสอบถามข้อมูลส่วนบุคคล การรับรู้ปัญหาการดูแลสุขภาพเด็ก และการเข้าถึงบริการสุขภาพของเด็กจำนวน 121 ราย และทำการสนทนากลุ่มกับผู้ดูแล 13 ราย รวมจำนวน 134 ราย คัดเลือกแบบเจาะจงและเก็บข้อมูลระหว่างเดือนตุลาคม 2561 – กันยายน 2562 วิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติบรรยายและข้อมูลเชิงคุณภาพโดยการวิเคราะห์เชิงเนื้อหา
ผลการวิจัย: ผู้ดูแลมีการรับรู้ปัญหาการดูแลสุขภาพของเด็กโดยรวมอยู่ในระดับต่ำ (ร้อยละ 80.20) ในขณะที่มีรับรู้การเข้าถึงบริการสุขภาพของเด็กโดยรวมอยู่ในระดับสูง (ร้อยละ 75.20) ข้อมูลเชิงคุณภาพสะท้อนปัญหาการดูแลสุขภาพเด็กใน 3 ประเด็นหลัก คือ 1) ดูแลยากหลากปัญหา ต้องมาพบแพทย์บ่อย 2) มีปัญหาด้านจิตใจ คิดอยากให้สังคมเข้าใจและยอมรับ และ 3) ค่าใช้จ่ายในการดูแลและการเดินทางสูง ส่วนการเข้าถึงบริการสุขภาพของเด็ก 2 พบประเด็นหลัก คือ 1) ข้อมูลของโรงพยาบาลและความเชี่ยวชาญของแพทย์คือสิ่งชี้นำการตัดสินใจ และ 2) มุมมองและเครือข่ายของทีมสุขภาพมีผลต่อหนทางการรักษา
ข้อเสนอแนะ : บุคลากรทีมสุขภาพควรช่วยเหลือเด็กที่มีความพิการบนใบหน้าและกะโหลกศีรษะแต่กำเนิดที่มีปัญหาสุขภาพรุนแรงและซับซ้อนให้เข้าถึงบริการสุขภาพ โดยการส่งต่อเพื่อมารับการรักษาในโรงพยาบาลที่มีศูนย์แก้ไขความพิการบนใบหน้าและกะโหลกศีรษะตั้งแต่ระยะแรก
