ความชุกและปัจจัยทำนายภาวะกล้ามเนื้อน้อยในผู้สูงอายุในชุมชน: การวิเคราะห์ข้อมูลทุติยภูมิ

บทนำ ภาวะมวลกล้ามเนื้อน้อยเป็นการสูญเสียมวลกล้ามเนื้อ ความแข็งแรง และการทำงานของกล้ามเนื้ออย่างค่อยเป็นค่อยไป มักพบในผู้สูงอายุโดยเฉพาะผู้ที่มีโรคร่วม ส่งผลให้เกิดความเสี่ยงต่อการหกล้ม และความพิการเพิ่มขึ้น และคุณภาพชีวิตลดลง ภาวะมวลกล้ามเนื้อน้อยมักไม่แสดงอาการในระยะเริ่มแรก หากสามารถตรวจพบได้ตั้งแต่ระยะเสี่ยง จะช่วยให้ผลลัพธ์ด้านสุขภาพที่ไม่พึงประสงค์ลดลง อย่างไรก็ตามยังขาดข้อมูลเชิงประจักษ์เกี่ยวกับภาวะมวลกล้ามเนื้อน้อยในผู้สูงอายุที่อาศัยอยู่ในชุมชนเมือง โดยเฉพาะกรุงเทพมหานคร อีกทั้งยังไม่มีแบบจำลองเชิงพยากรณ์ที่สามารถใช้คัดกรองผู้สูงอายุในระดับปฐมภูมิ
วัตถุประสงค์ การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาความชุกและปัจจัยทำนายภาวะมวลกล้ามเนื้อน้อยในระดับความเสี่ยง ความเป็นไปได้ และการยืนยันภาวะมวลกล้ามเนื้อน้อยของผู้สูงอายุในชุมชนเมืองในเขตกรุงเทพมหานคร สมมติฐานการวิจัยคือ ปัจจัยส่วนบุคคล (อายุและเพศ) และปัจจัยด้านสุขภาพ (โรคร่วม ดัชนีมวลกาย และเส้นรอบเอว) สามารถร่วมกันทำนายความเสี่ยงต่อภาวะมวลกล้ามเนื้อน้อย ความเป็นไปได้ของมวลกล้ามเนื้อน้อยและภาวะมวลกล้ามเนื้อน้อยในผู้สูงอายุที่อาศัยอยู่ในชุมชนเมืองเขตกรุงเทพมหานคร
การออกแบบการวิจัย การศึกษาความสัมพันธ์เชิงทำนายในครั้งนี้เป็นการวิเคราะห์ข้อมูลทุติยภูมิจากผู้สูงอายุที่อาศัยอยู่ในชุมชนเมืองเขตกรุงเทพมหานครจำนวน 117 คน จากโครงการหลักที่ศึกษาทั่วประเทศ โดยการคัดกรองภาวะมวลกล้ามเนื้อน้อย ดำเนินการตามแนวทางของคณะทำงานของเอเชีย ด้านภาวะกล้ามเนื้อน้อยในปี พ.ศ. 2562 ซึ่งกำหนดวิธีประเมินความแข็งแรงของกล้ามเนื้อ มวลกล้ามเนื้อ และ สมรรถภาพทางกาย
วิธีดำเนินการวิจัย ผู้วิจัยเลือกตัวอย่างแบบเฉพาะเจาะจง ตามเกณฑ์คัดเข้าคือ มีข้อมูลที่สมบูรณ์ในการประเมินภาวะมวลกล้ามเนื้อน้อยทั้ง 3 ตัวชี้วัดคือ เส้นรอบน่อง แรงบีบมือ และระยะเวลาในการลุก-นั่งเก้าอี้ 5 ครั้ง เก็บรวบรวมข้อมูลโดยใช้แบบบันทึก วิเคราะห์ข้อมูลใช้สถิติเชิงบรรยายและ Multinomial logistic regression เพื่อศึกษาปัจจัยทำนายภาวะมวลกล้ามเนื้อน้อย ทั้งสามระดับ ได้แก่ ความเสี่ยงต่อภาวะมวลกล้ามเนื้อน้อย ความเป็นไปได้ของภาวะมวลกล้ามเนื้อน้อย และภาวะมวลกล้ามเนื้อน้อย
ผลการวิจัย ผู้สูงอายุในชุมชนส่วนใหญ่เป็นเพศหญิง (ร้อยละ 68.4) อายุเฉลี่ย 69.7 ปี (S.D. = 7.2) ส่วนใหญ่มีโรคร่วม (ร้อยละ 70.9) โดยโรคที่พบมากที่สุด 3 อันดับแรก ได้แก่โรคความดันโลหิตสูง (ร้อยละ 58.1) รองลงมาคือ โรคเบาหวาน (ร้อยละ 37.6) และไขมันในเลือดสูง (ร้อยละ 15.4) ดัชนีมวลกายเฉลี่ย 24.69 กก/ตรม. (S.D. = 5.31) เพศหญิงส่วนใหญ่มีเส้นรอบเอวเกินเกณฑ์ (ร้อยละ 70.3) เพศชายส่วนใหญ่มีเส้นรอบเอวปกติ (ร้อยละ 54.0) ความชุกของความเสี่ยงต่อภาวะมวลกล้ามเนื้อน้อย (เส้นรอบน่องต่ำ) ความเป็นไปได้ของภาวะมวลกล้ามเนื้อน้อย (เส้นรอบน่องต่ำ ร่วมกับแรงบีบมือต่ำหรือระยะเวลามากในการลุกขึ้นยืนจากท่านั่ง 5 ครั้ง) และภาวะมวลกล้ามเนื้อน้อย (เส้นรอบน่องต่ำร่วมกับแรงบีบมือต่ำและระยะเวลามากในการลุกขึ้นยืนจากท่านั่ง 5 ครั้ง) ในผู้สูงอายุ เท่ากับร้อยละ 43.6, 12.7 และ 28.2 ตามลำดับ ซึ่งบ่งชี้ว่าผู้สูงอายุที่อาศัยอยู่ในชุมชนเกือบครึ่งหนึ่ง มีความเสี่ยงต่อภาวะมวลกล้ามเนื้อน้อย ผลการวิเคราะห์เชิงทำนายพบว่า ปัจจัยทำนายความเสี่ยง ต่อภาวะมวลกล้ามเนื้อน้อยได้อย่างมีนัยสำคัญทางสถิติ คือ ดัชนีมวลกาย (OR= 0.672, 95% C1: 0.488 - 0.926,p = .015) ปัจจัยทำนายความเป็นไปได้ของภาวะมวลกล้ามเนื้อน้อย คือ ดัชนีมวลกาย (OR= 0.775, 95% CI: 0.641 - 0.937, p = .009) ปัจจัยทำนายภาวะมวลกล้ามเนื้อน้อย ได้แก่ อายุ(OR = 1.094,95% CI:1.017-1.177,p=.016) และดัชนีมวลกาย (OR=0.736;95% C1: 0.626-0.865, p < .001)
ข้อเสนอแนะ ผลการศึกษานี้ชี้ให้เห็นถึงความจำเป็นในการตรวจคัดกรองตั้งแต่ระยะเสี่ยง โดยเฉพาะในผู้สูงอายุที่มีดัชนีมวลกายต่ำ นอกจากนี้การวางแผนพัฒนาโปรแกรมเสริมสร้างความแข็งแรงของกล้ามเนื้อและสมรรถภาพทางกายตั้งแต่ระยะแรกจะช่วยลดผลกระทบของภาวะมวลกล้ามเนื้อน้อย และส่งเสริมคุณภาพชีวิตโดยรวมของผู้สูงอายุในชุมชนเมืองเขตกรุงเทพมหานคร
