ประเด็นขัดแย้งทางจริยธรรมของพยาบาลที่ปฏิบัติงานในแผนกฉุกเฉิน โรงพยาบาลระดับตติยภูมิ

ประเด็นขัดแย้งทางจริยธรรม (Ethical dilemmas [EtD]) เกิดขึ้นกับพยาบาลเมื่อผู้รับบริการไม่ได้มีความต้องการและเป้าหมายตรงกับทีมให้การรักษา วิจัยเชิงบรรยายนี้มีวัตถุประสงค์เพื่อศึกษาผลกระทบ และความถี่ของ EtD ของพยาบาลจากสถานการณ์ที่กำหนด ผู้เข้าร่วมการวิจัยเป็นพยาบาลจำนวน 136 คน ที่ได้รับเลือกตามเกณฑ์ คือ ปฏิบัติงานในแผนกฉุกเฉินของโรงพยาบาล 5 แห่งในภาคใต้ มากกว่า 1 ปี สถานการณ์ที่กำหนด คือ เมื่อผู้ป่วย และ/หรือ ญาติ 1) ปฏิเสธการรักษา 2) ไม่ฉุกเฉินแต่ต้องการรับบริการแบบฉุกเฉิน 3) จำเป็นต้องรับการผ่าตัดด่วนแต่มีปัญหาค่ารักษาพยาบาล และ 4) ได้รับคำสั่งให้ยุติการช่วยฟื้นคืนชีพ ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหา โดยผู้ทรงคุณวุฒิ 3 ท่าน (ได้ค่าความตรงเชิงเนื้อหาเท่ากับ .90) เครื่องมือที่ใช้เก็บข้อมูลมี 2 ส่วน คือ แบบสอบถามข้อมูลส่วนบุคคล และแบบสอบถามความถี่และผลกระทบของประเด็นขัดแย้งทางจริยธรรม ทดสอบความเที่ยงด้วยสัมประสิทธิ์แอลฟาของครอนบาก ได้เท่ากับ .91 วิเคราะห์ข้อมูลด้วยสถิติแบบบรรยาย ผลการวิจัย พบว่า ผลกระทบของ EtD ในสถานการณ์ที่ 4 มีค่าเฉลี่ยมากที่สุด (M = 7.68, SD = 1.55) ซึ่งอยู่ในระดับมาก ส่วนความถี่ของการเผชิญกับ EtD ที่เกิดขึ้นมากที่สุด ในสถานการณ์ที่ 1 และ 2 คือ บางครั้ง (ร้อยละ 44.9 และ 39.7 ตามลำดับ) ในสถานการณ์ที่ 3 และ 4 คือ นาน ๆ ครั้ง (ร้อยละ 39.0 และ 40.4 ตามลำดับ) โดยสรุปแล้วพยาบาลประเมินว่าสถานการณ์การได้รับคำสั่งให้ยุติการช่วยฟื้นคืนชีพมีค่าเฉลี่ยผลกระทบจาก EtD มากที่สุดและมีความถี่ในการเกิดน้อยที่สุด คือ นาน ๆ ครั้ง เท่านั้น ส่วนความถี่ในการเกิด EtD ที่พบมากที่สุด คือ บางครั้ง จึงสามารถนำผลนี้มาประกอบการออกแบบการอบรมเชิงปฏิบัติการสำหรับพยาบาลที่ปฏิบัติงานในแผนกฉุกเฉินเพื่อพัฒนาทักษะการวิเคราะห์ EtD และสมรรถนะจริยธรรมทางการพยาบาล
