The Development of a Nursing Care Model for Patients with Intracerebral Aneurysm Undergoing Aneurysm Clipping Surgery

Aneurysm clipping surgery for persons with ruptured intracerebral aneurysms and quality care prevent disability and save the patient's life. This research and development project aimed to 1) develop clinical practice guidelines for patients with intracerebral aneurysms undergoing aneurysm clipping surgery (CPG-IAACS), 2) develop nurses’ competencies for caring for those patients, and 3) determine the effects of the nursing care model. There were 4 phases in this study which 1) involved studying the clinical problems and context with 15 participants who were operating room (OR) nurses and the drafting the CPG-IAACS, 2) the CPG-IAACS was assessed and it was found that its overall quality, according to the AGREE II criteria, was 94.6%, so it was suitable for use, 3) focused on the revision of the CPG-IAACS in conjunction with improving OR nurses’ competencies using the GROW coaching technique, and 4) was the implementation and evaluation of the revised CPG-IAACS with 10 OR nurses who were coachers, 9 OR nurses who were coached, and 9 patients had undergone aneurysm clipping who were cared for using CPG-IAACS. Data was collected using a nurses' competency assessment form with a scale-content validity index (S-CVI) of .90 and a reliability of .88. The nursing outcomes on the client's record form had a S-CVI of 1.00. The data were analyzed using descriptive statistics and the Wilcoxon signed rank tests. The results showed that after implementing the CPG-IAACS, 1 patient had a complication, the mean length of stay in the hospital was 14.75 days (SD = 9.85) and 1 patient died. Nurse coaches and OR nurses who were coached had significantly different mean competency scores before and after the training (z = -2.23, p < .05, and z = -2.38, p < .05, respectively). To achieve positive patient outcomes, CPG-IAACS should be consistently employed and the competencies of nurses should be systematically improved.
