Development of a Motivational Interviewing Model to Promote Health Literacy and Behavior Change for Stroke Prevention among High-Risk Groups in Primary Care

Abstract

Stroke is a major public health issue, particularly among patients with risk factors such as hypertension, diabetes, and hyperlipidemia. These groups often lack adequate knowledge and motivation to modify their health behaviors, resulting in complications, premature disability and death. This research and development project aimed to: (1) examine the current situation and challenges in stroke prevention among high-risk patients in primary care settings; (2) develop a motivational interviewing model to promote health behavior change and enhance health literacy for stroke prevention in high-risk patients; and (3) evaluate the effectiveness of the developed model. The study involved 32 participants including administrators, healthcare providers, and network partners, as well as 30 high-risk patients. The research was conducted in three phases using both qualitative and quantitative approaches. Phase 1 involved in-depth interviews with administrators, providers, and partners to analyze the current situation and identify problems. Phase 2 focused on developing the motivational interviewing model which incorporated health literacy enhancement, counseling, follow-up, and evaluation. Phase 3 assessed the effectiveness of the model through a 12-week implementation using a one-group pretest-posttest design. Data were analyzed using content analysis and paired t-tests. The results showed statistically significant improvements in both health literacy and health behaviors among high-risk patients after the intervention (t = 21.963 and t = 8.712, respectively; p < .001), indicating the effectiveness of the developed model in promoting stroke prevention at the primary care level.
