Side effects and first year Adherence of HIV infected patients in Wapipathum hospital.
Abstract
Background : Acquired Immune Deficiency Syndrome (AIDS) is caused by the HIV virus. When HIV infectionisadvanced, the HIV- infected patient immunesystemwasweaked until theother infection developed. There is currently no cure for HIV.However with antiviral drug, patient can live longer and healthy with HIV. The drug level needs to high enough for viral suppression unless the viral will develope drug resistant virus. Drug adherenceis animportant factor for HIVtreatment.
Objectives :Tostudythefactors,especiallysideeffectof Antiviraland opportunisticinfectionsmedication, that affect the patient adherenceinthefirst yearof treatment in Wapipathum hospital.
Method : Descriptive Cross-sectionalStudyof HIV-infected caseinWapipathumhospital from1st October 2013 - 30th September 2015. From 112 cases whose diagnosed HIV infection , 40 cases were first year antiviral drugtreatment. Mostcomplainingside-effects is dizziness with53%. Others sideeffectsarerash(25%), diarrhea (23%)hepatitis (15%),and flushing(13%) respectively.Thecomparison betweenthegood adherencegroup and the non - adherence group was done. Dizziness (59% vs 25%), hepatitis (16% vs 13%), flushing (16%vs0%) are more ingood adherencegroup.Rashis complaint equally in bothgroup (25%). Diarrheahas more complaining innon- adhereancegroup (19%vs38%) Over all opportunistic infection is tuberculosis with 30%. oral candidiasis ,pruritic purpuraeruption, Pneumocystisjerovecipneumonia,cryptococcus infection,and Herpeszosters infectionare18,18,13,8,and 8%. The percentageofopportunistic infections was compared betweenthe good adherence group and the non-adherencegroup.Tuberculosisinfection(31%vs25%), pruritic papulareruptionrash (19% vs13%)aremore ingood adherencegroup.Pneumococcusjeroveci infectionareequalat13%.Theinfections that found morein pooradherencegroup areoralcandidiasis (16% vs25%),cryptococcus infection(6% vs13%)and Herpeszoster infection(6% vs13%).
Conclusions : Sideeffectsof Antiviraland opportunisticinfectionsmedicationare partofmultifactor that affects the patient adherence.
