DIAGNOSIS AND MANAGEMENT OF BURNING MOUTH SYNDROME ASSOCIATED WITH HYPOSALIVATION IN ELDERLY: A CASE REPORT

Burning mouth syndrome (BMS) is a chronic neuropathic orofacial pain disorder that severely impairs patients' quality of life. This case report presents the diagnosis and management of a 78-year-old Thai female at the Dental Clinic. She presented with a severe burning sensation on her oral cavity lasting for 8 months. These symptoms were accompanied by objective hyposalivation and dry mouth, without any apparent mucosal lesions or atrophic glossitis.
Laboratory investigations revealed severe iron deficiency anemia, Esophagogastroduodenoscopy and colonoscopy showed no sources of chronic gastrointestinal bleeding, suggesting that the condition was caused by dietary deficiency and age-related malabsorption. The patient was managed systemically with ferrous fumarate and folic acid, combined with localized salivary gland massage, and neuropathic pain medications were deliberately avoided.
Within 3 months, the oral burning symptoms completely resolved, her complete blood count and ferritin levels returned to normal. This case demonstrates that burning mouth symptoms can be the primary manifestation of iron deficiency anemia despite normal-appearing oral mucosa. Accurate differential diagnosis and targeted etiological treatment can achieve complete disease remission without the need for neurotropic medications.
