บทคัดย่อ
โรคหลอดเลือดสมองเป็นปัญหาสาธารณสุขที่สำคัญของประเทศไทย โดยมีจำนวนผู้ป่วยมากกว่า 300,000 รายต่อปี และมีอัตราการเสียชีวิตกว่า 50,000 รายต่อปี ส่งผลกระทบต่อระบบบริการสุขภาพ เศรษฐกิจ และคุณภาพชีวิตของผู้ป่วยและครอบครัว หลอดเลือดแดงคาโรติดตีบตัน (Carotid Artery Stenosis; CAS) เป็นหนึ่งในปัจจัยเสี่ยงสำคัญของโรคหลอดเลือดสมองชนิดขาดเลือด โดยเฉพาะในผู้สูงอายุที่มีโรคร่วมหลายระบบ การผ่าตัดเอาสิ่งอุดตันออกจากหลอดเลือดแดงคาโรติด (Carotid Endarterectomy; CEA) เป็นหัตถการมาตรฐานที่มีประสิทธิภาพในการป้องกันการเกิดโรคหลอดเลือดสมองซ้ำ ช่วยลดอัตราการเกิดอัมพาตถาวร และเพิ่มอัตราการรอดชีวิต อย่างไรก็ตาม การผ่าตัดนี้มีความซับซ้อนรวมทั้งมีความเสี่ยงต่อการเกิดภาวะแทรกซ้อน เช่น ภาวะสมองขาดเลือดเฉียบพลัน (stroke) กล้ามเนื้อหัวใจตาย (myocardial infarction) และภาวะเลือดออกหรือก้อนเลือดกดทับ (hematoma) เป็นต้น ดังนั้น การดูแลทางการพยาบาลจึงต้องการความเชี่ยวชาญเฉพาะทาง ความแม่นยำในการประเมิน และการประสานงานแบบสหสาขาวิชาชีพ บทความทบทวนนี้มีวัตถุประสงค์เพื่อนำเสนอแนวทางการพยาบาลที่ครอบคลุมการประเมิน เฝ้าระวัง และดูแลผู้ป่วยในทุกระยะของการผ่าตัด CEA เพื่อเป็นแนวทางเชิงปฏิบัติที่ชัดเจน ส่งเสริมผลลัพธ์การรักษาที่ดี ลดความเสี่ยงของภาวะแทรกซ้อน และเพิ่มคุณภาพการดูแลผู้ป่วยอย่างมีประสิทธิภาพ
ปัจจุบันแนวปฏิบัติการพยาบาลสำหรับผู้ป่วย CEA ยังมีความหลากหลายระหว่างสถานพยาบาล ส่งผลให้คุณภาพการดูแลไม่เป็นมาตรฐานเดียวกัน บทความนี้จึงนำเสนอแนวทางการพยาบาลที่เป็นระบบ และมีหลักฐานเชิงประจักษ์ ครอบคลุมการดูแลตลอดระยะผ่าตัด ตั้งแต่การประเมิน และเตรียมผู้ป่วยก่อนผ่าตัด การเฝ้าระวังและจัดการภาวะแทรกซ้อนระหว่างผ่าตัด จนถึงการดูแลฟื้นฟูหลังผ่าตัด เพื่อส่งเสริมผลลัพธ์ทางการผ่าตัดที่ดี และความปลอดภัยของผู้ป่วย
