Factors Related to Respiratory Failure Requiring Invasive Mechanical Ventilator (MV) in Patients with Acute Heart Failure

Abstract

This retrospective study aimed to identify factors related to respiratory failure requiring invasive mechanical ventilator (MV) in patients with acute heart failure. Medical records of 300 patients diagnosed with heart failure who were admitted with acute exacerbation at Bhumibol Adulyadej Hospital, Directorate of Medical Services, Royal Thai Air Force, between October 1, 2023, and September 30, 2024, were reviewed. The variables examined included personal factors and health and illness-related factors. Factors related to respiratory failure requiring invasive mechanical ventilator (MV) in patients with acute heart failure were analyzed using logistic regression.

The results revealed that significant factors related to respiratory failure requiring invasive mechanical ventilator (MV) in patients with acute heart failure at a statistically significant level of .05 were as follows: body mass index in the range of 25.0–29.9 (OR 25.17, 95% CI 1.17–543.60), body mass index in the range of 30.0–39.9 (OR 37.49, 95% CI 1.57–894.69), respiratory tract infection (OR 0.03, 95% CI 0.01–0.18), and NYHA Class IV heart failure severity (OR 0.02, 95% CI 1.03–2.50). In order to reduce the incidence of respiratory failure requiring invasive mechanical ventilator in patients with acute heart failure, healthcare professionals should prioritize body mass index reduction, thorough assessment of heart failure severity, and active screening for respiratory tract infections.
