บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาเปรียบเทียบการตรวจหาเชลล์มะเร็งปากมดลูก โดยวิธีการตรวจด้วยตนเอง โดยใช้แปรงสีฟันสำหรับเด็กอ่อน กับวิธี Pap smear ตามมาตรฐาน
รูปแบบการวิจัย: วิจัยแบบไปข้างหน้า โดยวิธี Diagnostic test
สถานที่ศึกษา: แผนกผู้ป่วยนอก กลุ่มงานสูตินรีเวชวิทยา และวางแผนครอบครัวโรงพยาบาล ศูนย์สุรินทร์
กลุ่มตัวอย่าง: สตรีที่มารับการตรวจ colposcope ที่โรงพยาบาลศูนย์สุรินทร์ ระหว่างวันที่ 8 มกราคม พ.ศ. 2550 ถึง 25 กุมภาพันธ์ 2551 จำนวน 220 ราย
ผลการศึกษา: จากการตรวจด้วยแปรงสีฟันเด็กอ่อนให้ผล satisfactory presence endocervical gland ต่ำกว่า Pap smear (ร้อยละ 31.37 และ 94.5) ซึ่งแตกต่างอย่างมี นัยสำคัญ (p < 0.01) การตรวจด้วยแปรงสีฟันเด็กอ่อนให้ผล sensitivity ต่ำกว่า Pap smear (ร้อยละ 26.3 และ 33) ให้ผล false negative สูงกว่า Pap smear (ร้อยละ 73.7 และ 66.9) ซึ่งแตกต่างอย่างมีนัยสำคัญ (p < 0.01) แต่ให้ผล specificity สูงกว่า Pap smear (ร้อยละ 96.5 และ 80.4) ให้ผล positive predictive value สูงกว่า Pap smear (ร้อยละ 92.1 และ 72.1) ซึ่งแตกต่าง อย่างมีนัยสำคัญ(p < 0.01)
สรุป: การตรวจด้วยตนเองโดยใช้แปรงสีฟันเด็กอ่อนให้ผลความแม่นยำแตกต่างไม่มาก จากวิธี Pap smear แต่เนื่องจากแปรงสิฟันเด็กอ่อน หาชื้อได้ง่าย นำกลับมาใช้ ได้บ่อย ๆ การตรวจถี่ขึ้นอาจจะช่วยเพิ่มความแม่นยำในการวินิจฉัยได้
