Sensitivity of Graham's Score for Appendicitis Complication Screening in Buriram Hospital

Abstract
Background: Acute appendicitis is the most common cause of acute abdominal pain. Delayed in diagnosis and operation may resulted in complications such as perforation and gangrene. Appendicitis complication increases morbidity and mortality, thus Graham's clinical scoring index was developed for screening appendicitis complication.
Objective: To evaluate Graham's score sensitivity for appendicitis complication screening
Study design: Retrospective study
Method: Patient's charts diagnosed with acute appendicitis and underwent appendectomy during 1st August - 30th December 2007 were reviewed. Patient demographic data and seven predictive symptoms were collected and assessed by Graham's score to verify its sensitivity, specificity, positive predictive value and negative predictive value.
Result: From 363 patient's charts assessed by Graham's score for appendicitis complication screening found that its sensitivity and specificity were 76.59% and 42.34%. While the positive predictive value and negative predictive value were 45.7% and 74.01% respectively.
Conclusion: Graham's score had low sensitivity in appendicitis complication screening according to early admission, severity of abdominal pain and unclear subjective data which affected its accuracy.
