บทคัดย่อ
nonoperative management ใน blunt hepatic injury ในปัจจุบัน ได้รับการยอมรับและ ใช้อย่างแพร่หลาย โดยเฉพาะในสถาบันใหญ่ๆ มีอัตราการเข้าสู่ nonoperative management สูง ในขณะที่พบภาวะแทรกช้อนและการเกิด failure ค่อนข้างต่ำ สาเหตุที่เกิด failure มากที่สุดคือ delayed hemorrhage ดังในรายงานผู้ป่วยรายนี้ ผู้ป่วยหญิงอายุ 37 ปี ประสบอุบัติเหตุ รถจักรยานยนต์ เนื่องจากมีข้อบ่งชี้คือ hemodynamic stable, ไม่มี peritoneal sign และได้ทำ CT scan พบมี subcapsular hematoma of liver จึงเข้าสู่ nonoperative management ในวันที่ 6 หลังอุบัติเหตุเกิด delayed hemorrgage ได้รับการผ่าตัดช่องท้อง ผู้ป่วยรายนี้ได้รับเลือดทั้งสิ้น 3 unit ระยะเวลานอนอยู่ในโรงพยาบาล 21 วัน
