มะเร็งไทรอยด์ชนิด Papillary และ Medullary ในผู้ป่วยรายเดียวกัน: รายงานผู้ป่วย

บทคัดย่อ
หลักการและเหตุผล: รายงานผู้ป่วยชายไทย อายุ 52 ปี มารักษาตัวที่โรงพยาบาลศรีสะเกษด้วยอาการ คลำพบก้อนที่บริเวณคอ ก้อนโตขึ้น มา 1 ปี ตรวจร่างกายพบไทรอยด์ฝั่งซ้ายโต ได้ทำการอัลตราซาวด์พบ ก้อนขนาด 4.7 เซนติเมตรที่ไทรอยด์ฝั่งซ้ายและก้อนขนาด 0.5 เซนติเมตรที่ไทรอยด์ฝั่งขวา (TI-RADS 4: moderately suspicious) ได้ทำการเจาะดูดเซลล์จากก้อนฝั่งซ้าย (fine needle aspiration) พบว่าเป็น Suspicious for papillary thyroid carcinoma ผู้ป่วยได้เข้ารับการผ่าตัดไทรอยด์ (total thyroidectomy) ในรายงานฉบับนี้ได้นำเสนอตัวอย่างเคสผู้ป่วยและอธิบายลักษณะที่พบทางพยาธิวิทยาของมะเร็งไทรอยด์ชนิด Papillary thyroid carcinoma และ Medullary thyroid microcarcinoma ในผู้ป่วยรายเดียวกัน ซึ่งมะเร็งทั้ง 2 ชนิดนี้พบได้ร่วมกันน้อยมาก และงานทางพยาธิกายวิภาคมีบทบาทสำคัญในการตรวจวินิจฉัย
