บทคัดย่อ
หลักการและเหตุผล: การลดน้ำหนักร้อยละ 5-10 ลดความเสี่ยงโรคเบาหวานชนิดที่ 2 และหัวใจได้อย่างมีนัยสำคัญ แอปพลิเคชันสุขภาพบนมือถือ (mHealth) มีศักยภาพสูงเนื่องจากต้นทุนต่ำและเข้าถึงง่าย แต่ยังมีหลักฐานจำกัดและข้อสรุปที่ยังไม่ชัดเจนเกี่ยวกับความปลอดภัยด้านพฤติกรรมการบริโภคและความเสี่ยงต่อโรคการกินผิดปกติ จึงจำเป็นต้องมีการประเมินอย่างเป็นระบบ
วัตถุประสงค์: เพื่อประเมินประสิทธิภาพและความปลอดภัยของแอปพลิเคชันสุขภาพบนมือถือในผู้ใหญ่ที่มีภาวะอ้วน
วิธีการศึกษา: ทบทวนวรรณกรรมตามแนวทาง PRISMA 2020 สืบค้นฐานข้อมูล PubMed ระหว่างปีค.ศ. 2015-2025 พบ 91 บทความ ได้การศึกษาแบบ RCT 9 ฉบับ (N=3,633) มีข้อมูลเพียงพอสำหรับการวิเคราะห์อภิมาน 6 ฉบับ วิเคราะห์ค่าความแตกต่างเฉลี่ย (MD) ด้วยแบบจำลอง Random-effects และประเมินความเชื่อมั่นของหลักฐานด้วย Grading of Recommendations Assessment, Development and Evaluation (GRADE)
ผลการศึกษา: กลุ่มที่ใช้แอปพลิเคชันมีน้ำหนักลดลงมากกว่ากลุ่มควบคุม -2.18 kg (95% CI: -3.47 to -0.89; p=0.001, I²=76%) และมีโอกาสลดน้ำหนักได้ไม่น้อยกว่าร้อยละ 5 สูงกว่า 1.53 เท่า โดยแอปพลิเคชันที่ผสมผสานการออกกำลังกายและการจัดการอาหารร่วมกันมีประสิทธิภาพสูงกว่า (MD -2.84 kg) นอกจากนี้ยังช่วยเพิ่มกิจกรรมทางกายและลดพลังงานที่บริโภค ไม่พบเหตุการณ์ไม่พึงประสงค์ร้ายแรง และช่วยลดอาการของโรคการกินผิดปกติ (OR 0.51)
สรุป: แอปพลิเคชันสุขภาพบนมือถือมีประสิทธิภาพและมีความปลอดภัยในการลดน้ำหนักและปรับเปลี่ยนพฤติกรรมสุขภาพในผู้ใหญ่ที่มีภาวะอ้วนโดยมีระดับความเชื่อมั่นของหลักฐานปานกลาง (moderate certainty, GRADE)
