การวางแผนการรักษาทางทันตกรรมภายใต้การดมยาสลบในผู้ป่วยเด็ก โรคหัวใจพิการแต่กำเนิดชนิดฟัลโลต์เตตระโลจี: รายงานกรณีศึกษา

บทคัดย่อ
ฟัลโลต์เตตระโลจีเป็นโรคหัวใจพิการแต่กำเนิดชนิดเขียวที่มีความซับซ้อนและมีความเสี่ยงสูงต่อการติดเชื้อที่เยื่อบุหัวใจจากเชื้อในช่องปาก รายงานผู้ป่วยนี้มีวัตถุประสงค์เพื่อนำเสนอการวางแผนและการรักษาทางทันตกรรมภายใต้การดมยาสลบในผู้ป่วยเด็กที่มีโรคดังกล่าว โดยการดูแลแบบสหสาขาวิชาชีพ
ผู้ป่วยเด็กชายไทยอายุ 4 ปี 3 เดือน มีโรคประจำตัวเป็นฟัลโลต์เตตระโลจีที่ยังไม่ได้รับการผ่าตัดแก้ไข ร่วมกับมีความผิดปกติของทวารหนัก ไต และระบบโลหิตวิทยา มาด้วยปัญหาฟันผุหลายซี่และไม่ให้ความร่วมมือในการรักษา จึงวางแผนการรักษาแบบบูรณะทั้งปากภายใต้การดมยาสลบ โดยมีการประเมินความเสี่ยงร่วมกับกุมารแพทย์โรคหัวใจและวิสัญญีแพทย์ พร้อมทั้งให้ยาปฏิชีวนะเพื่อป้องกันการติดเชื้อที่เยื่อบุหัวใจก่อนเริ่มหัตถการ การรักษาทางทันตกรรมประกอบด้วย อุดฟัน ครอบฟัน และถอนฟันหลายซี่ ไม่พบภาวะแทรกซ้อนระหว่างทำหัตถการ ในระยะพักฟื้นที่หอผู้ป่วย พบภาวะอุดกั้นทางเดินหายใจ ส่งผลให้ระดับความอิ่มตัวของออกซิเจนเหลือร้อยละ 60 โดยสามารถตรวจจับและแก้ไขจนผู้ป่วยปลอดภัย
ผลการรักษา สามารถกำจัดแหล่งติดเชื้อในช่องปากได้อย่างมีประสิทธิภาพ ช่วยเตรียมความพร้อมให้ผู้ป่วยเข้ารับการผ่าตัดหัวใจได้อย่างปลอดภัย การรักษาทันตกรรมภายใต้การดมยาสลบจึงเป็นทางเลือกที่เหมาะสมสำหรับผู้ป่วยที่มีความซับซ้อนของโรคและไม่ให้ความร่วมมือ ทั้งนี้ต้องวางแผนและการดูแลผู้ป่วยร่วมกันของทีมสหสาขาวิชาชีพอย่างมีประสิทธิภาพ รวมถึงการติดตามและส่งเสริมป้องกันโรคฟันผุตามแนวทางกลุ่มผู้ป่วยที่มีความเสี่ยงฟันผุสูง
