ผลลัพธ์และความปลอดภัยของการให้ยา Streptokinase ขนาดครึ่งหนึ่งในผู้ป่วยสูงอายุที่มีภาวะกล้ามเนื้อหัวใจตายเฉียบพลันชนิด ST-segment Elevation ในเครือข่ายจังหวัดศรีสะเกษ

บทคัดย่อ
บทนำ: ภาวะกล้ามเนื้อหัวใจตายเฉียบพลันชนิด STEMI ในผู้สูงอายุมีความเสี่ยงสูงต่ออัตราการเสียชีวิตและภาวะแทรกซ้อน แม้การให้ยาละลายลิ่มเลือด Streptokinase (SK) ขนาดมาตรฐาน (1,500,000 ยูนิต) จะมีประสิทธิภาพดี แต่สัมพันธ์กับความเสี่ยงภาวะเลือดออกในสมองที่สูงขึ้นในผู้ป่วยกลุ่มนี้ การใช้ยา SK ขนาดครึ่งหนึ่ง (750,000 ยูนิต) จึงถูกนำมาใช้เพื่อเพิ่มความปลอดภัย แต่ข้อมูลเปรียบเทียบในบริบทประเทศไทยยังมีจำกัด
วัตถุประสงค์: เพื่อเปรียบเทียบผลลัพธ์ทางคลินิก ประสิทธิภาพ และความปลอดภัยของการให้ยา SK ขนาดครึ่งหนึ่งเทียบกับขนาดมาตรฐานในผู้ป่วยสูงอายุที่มีภาวะ STEMI ณ โรงพยาบาลศรีสะเกษ
วิธีการศึกษา: การศึกษาวิจัยเชิงพรรณนาแบบย้อนหลังในผู้ป่วยอายุ 70 ปีขึ้นไป ที่ได้รับยา SK ณ โรงพยาบาลทุกแห่งในจังหวัดศรีสะเกษ ระหว่างวันที่ 1 ตุลาคม พ.ศ. 2566 ถึง 31 สิงหาคม พ.ศ. 2568 ประเมินตัวชี้วัดหลักคืออัตราความสำเร็จในการเปิดหลอดเลือดที่ 60 นาที ตัวชี้วัดรองได้แก่ อุบัติการณ์เลือดออกรุนแรงและอัตราการเสียชีวิตในโรงพยาบาล วิเคราะห์ข้อมูลด้วยวิธี Propensity Score Matching (PSM) เพื่อลดปัจจัยรบกวน
ผลการศึกษา: จากผู้ป่วยทั้งหมด 98 ราย พบผู้ได้รับยา SK ขนาดครึ่งหนึ่งร้อยละ 37 ผลการศึกษาพบว่ากลุ่มขนาดครึ่งหนึ่งมีอัตราการเปิดหลอดเลือดสำเร็จร้อยละ 62.9 เทียบกับกลุ่มขนาดมาตรฐานร้อยละ 54.3 (p = 0.467) ด้านความปลอดภัย พบอุบัติการณ์เลือดออกรุนแรงในกลุ่มขนาดครึ่งหนึ่งร้อยละ 8.3 ขณะที่กลุ่มขนาดมาตรฐานไม่พบอุบัติการณ์ดังกล่าว (p = 0.239) นอกจากนี้พบภาวะความดันโลหิตต่ำหลังได้รับยาในกลุ่มขนาดครึ่งหนึ่งน้อยกว่ากลุ่มขนาดมาตรฐาน (ร้อยละ 27.8 และ 50.0; p = 0.053) อย่างไรก็ตาม ความแตกต่างทั้งหมดไม่มีนัยสำคัญทางสถิติ
สรุป: ประสิทธิภาพและความปลอดภัยของการให้ยา SK ขนาดครึ่งหนึ่งและขนาดมาตรฐานในผู้ป่วยสูงอายุที่เป็น STEMI ณ โรงพยาบาลศรีสะเกษ ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ
