บทคัดย่อ

หลักการและเหตุผล: มะเร็งเต้านมมีความสัมพันธ์กับการกลายพันธุ์ของยีน BRCA1 หรือ BRCA2 ซึ่งเพิ่มความเสี่ยงการเกิดมะเร็งซ้ำและก่อให้เกิดความวิตกกังวลในผู้ป่วย แม้ว่าการให้คำปรึกษาด้านยีนกลายพันธุ์อย่างเป็นระบบ จะมีส่วนช่วยลดความวิตกกังวลตลอดกระบวนการตรวจคัดกรอง แต่แนวปฏิบัติที่เป็นมาตรฐานในประเด็นดังกล่าวยังคงมีข้อจำกัดในบริบทของประเทศไทย


วัตถุประสงค์: เพื่อพัฒนาและประเมินผลการทดลองใช้แนวปฏิบัติการพยาบาลสำหรับการให้คำปรึกษาด้านยีนกลายพันธุ์ ต่อความรู้และความวิตกกังวลในผู้ป่วยมะเร็งเต้านมที่มีความเสี่ยงต่อการพบยีนกลายพันธุ์


วิธีการศึกษา: การพัฒนาแนวปฏิบัติการพยาบาลตามกรอบแนวคิดของ Soukup โดยดำเนินการ 3 ระยะ ได้แก่ วิเคราะห์สถานการณ์ ทบทวนหลักฐานและพัฒนาแนวปฏิบัติ และทดลองใช้แนวปฏิบัติในกลุ่มตัวอย่างผู้ป่วยมะเร็งเต้านมจำนวน 30 ราย ณ คลินิกมะเร็งและโรคทั่วไป โรงพยาบาลสุรินทร์ ประเมินผลโดยใช้แบบประเมินความรู้ด้านยีนกลายพันธุ์และแบบประเมินความวิตกกังวล และวิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Friedman test และ Wilcoxon signed-rank test


ผลการศึกษา: กลุ่มตัวอย่างมีคะแนนความรู้ พบว่า ก่อนได้รับคำปรึกษา คะแนนเฉลี่ย 14.5 ± 2.8 หลังได้รับคำปรึกษาทันที คะแนนเฉลี่ย 16.4 ± 1.8 และติดตามผล 1 เดือน คะแนนเฉลี่ย 18.2 ± 1.0 เมื่อเปรียบเทียบทั้ง 3 ระยะพบว่ามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (χ² = 34.137, P< 0.001) ส่วนคะแนนความวิตกกังวล พบว่า ก่อนได้รับคำปรึกษา คะแนนเฉลี่ย 38.4 ± 11.3 หลังได้รับคำปรึกษาทันที คะแนนเฉลี่ย 35.9 ± 11.6 และติดตามผล 1 เดือน คะแนนเฉลี่ย 24.8 ± 6.1 เมื่อเปรียบเทียบทั้ง 3 ระยะพบว่ามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (χ² = 14.708, p = 0.001)


สรุปผล: แนวปฏิบัติการพยาบาลที่พัฒนาขึ้นสามารถเพิ่มความรู้ด้านยีนกลายพันธุ์และลดความวิตกกังวลของผู้ป่วยมะเร็งเต้านมกลุ่มเสี่ยงต่อการพบยีนกลายพันธุ์ได้ อย่างไรก็ตาม ควรมีการศึกษาเพิ่มเติมในกลุ่มตัวอย่างขนาดใหญ่ขึ้น โดยมีกลุ่มควบคุมเพื่อเปรียบเทียบและยืนยันประสิทธิผลของแนวปฏิบัติดังกล่าว
