บทคัดย่อ

หลักการและเหตุผล: การฝังยาคุมกำเนิดมีประสิทธิภาพสูง แต่ผู้ใช้มักยุติก่อนกำหนดจากอาการข้างเคียง เช่น เลือดออกผิดปกติ ปวดบริเวณที่ฝัง ปัจจัยด้านอายุ และความต้องการมีบุตร ผู้วิจัยจึงศึกษาปัจจัยที่มีความสัมพันธ์กับการยุติการใช้ยาฝังคุมกำเนิดก่อนกำหนดในโรงพยาบาลบุรีรัมย์ เพื่อนำผลไปพัฒนาระบบบริการให้เหมาะสม


วัตถุประสงค์: เพื่อศึกษาอัตราการคงอยู่ของการใช้ยาฝังคุมกำเนิด และวิเคราะห์ปัจจัยที่มีความสัมพันธ์กับการยุติการใช้ก่อนครบกำหนด 3 ปี ในสตรีวัยเจริญพันธุ์ที่มารับบริการถอดยาฝังชนิด 3 ปี ณ โรงพยาบาลบุรีรัมย์


วิธีการศึกษา: เป็นการศึกษาเชิงวิเคราะห์ย้อนหลัง ผู้รับบริการที่คลินิกวางแผนครอบครัวโรงพยาบาลบุรีรัมย์ ที่มารับการยุติการใช้ยาฝังคุมกำเนิดชนิด 3 ปี ทั้งหมดในปี 2567 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และทดสอบความสัมพันธ์ด้วยสถิติ Chi-square, Fisher's Exact Test และ Logistic regression โดยกำหนดระดับนัยสำคัญทางสถิติที่ 0.05


ผลการศึกษา: ผู้ที่เข้ารับบริการยุติการใช้ยาฝังคุมกำเนิดชนิด 3 ปี ในปี 2567 ทั้งหมด 328 คน มี 91 คน ยุติการใช้ก่อนกำหนด เป็นกลุ่มอายุน้อยกว่า 20 ปี จำนวน 47 คน (ร้อยละ 51.7) โดยอายุมีความสัมพันธ์กับการยุติการใช้ก่อนกำหนดอย่างมีนัยสำคัญทางสถิติ (OR 0.874 95% CI: 0.808 – 0.946; p < 0.05) และปัจจัยที่เกี่ยวข้องคือ อาการข้างเคียงมีเลือดออกผิดปกติ (ร้อยละ 39.6) อาการปวดบริเวณที่ฝัง (ร้อยละ 24.2) โดยกลุ่มวัยรุ่นยุติจากอาการไม่พึงประสงค์ทางกายภาพ กลุ่มผู้ใหญ่มีการถอดเนื่องจากต้องการมีบุตรร่วมด้วย


สรุป: อายุเป็นปัจจัยที่มีความสัมพันธ์มากที่สุดกับการยุติการใช้ยาฝังคุมกำเนิดก่อนกำหนด โดยกลุ่มวัยรุ่นเป็นกลุ่มเสี่ยงสูงในการยุติการใช้ก่อนกำหนด โรงพยาบาลควรให้คำปรึกษา และติดตามอย่างใกล้ชิด โดยรักษาอาการข้างเคียงเบื้องต้น เพื่อเพิ่มอัตราการใช้ยาฝังคุมกำเนิดต่อเนื่อง
