บทคัดย่อ
หลักการและเหตุผล: ปัจจุบันเลเซอร์ Fractional CO2 เป็นหนึ่งในวิธีที่ได้รับความนิยมในการรักษาแผลเป็นหลุมสิว เนื่องจากให้ผลการรักษาที่ดีและมีผลข้างเคียงน้อย อย่างไรก็ตาม ยังไม่มีการศึกษาประสิทธิผลและความปลอดภัยของการรักษาแผลเป็นหลุมสิวด้วยเลเซอร์ Fractional CO2 ในโรงพยาบาลบุรีรัมย์
วัตถุประสงค์: เพื่อศึกษาประสิทธิภาพ ความปลอดภัย และปัจจัยทางคลินิกที่สัมพันธ์กับผลลัพธ์ของการรักษาแผลเป็นหลุมสิวด้วยเลเซอร์ Fractional CO2ในผู้ป่วยโรงพยาบาลบุรีรัมย์
วิธีการศึกษา: การศึกษาเชิงทดลองแบบแบ่งครึ่งใบหน้า (split-face comparative study) ในผู้ป่วยแผลเป็นหลุมสิว Fitzpatrick skin type III-IV 14 ราย ที่โรงพยาบาลบุรีรัมย์ โดยรักษาด้วยเลเซอร์ Fractional CO2 เปรียบเทียบระหว่างใบหน้าที่ใช้พลังงานมาตรฐานและพลังงานสูง รวมทั้งหมด 3 ครั้ง ห่างกันทุก 4–6 สัปดาห์ ประเมินผลการรักษาด้วยคะแนน ECCA ภาพถ่ายทางคลินิก อาการไม่พึงประสงค์ และวิเคราะห์ปัจจัยทางคลินิกที่สัมพันธ์กับผลลัพธ์การรักษา
ผลการศึกษา: ผู้เข้าร่วมวิจัย 14 ราย มีคะแนน ECCA เริ่มต้นของใบหน้าทั้งสองฝั่งใกล้เคียงกัน หลังการรักษาพบว่าฝั่งพลังงานสูงช่วยให้หลุมสิวดีขึ้นมากกว่าฝั่งควบคุมอย่างมีนัยสำคัญ โดยมีคะแนน ECCA ต่ำกว่าอย่างชัดเจน (61.8 ± 21.8 เทียบกับ 79.3 ± 28.5; paired t-test mean difference = -17.50, 95% CI: -26.68 ถึง -8.32; p = 0.001) นอกจากนี้ การวิเคราะห์ด้วย mixed-effects model พบว่าฝั่งพลังงานสูงมีอัตราการลดลงของคะแนน ECCA รวดเร็วกว่าฝั่งควบคุมอย่างมีนัยสำคัญ โดยเฉพาะในการรักษาครั้งที่ 3 (interaction coefficient = -18.93; p = 0.001) โดยหลุมสิวชนิดคลื่น (Rolling scars) ตอบสนองดีที่สุด โดยไม่พบภาวะแทรกซ้อนรุนแรงและไม่พบปัจจัยทางคลินิกอื่นที่สัมพันธ์กับผลการรักษาอย่างมีนัยสำคัญ
สรุป: การรักษาแผลเป็นหลุมสิวด้วยเลเซอร์ Fractional CO2 ด้วยพลังงานสูง มีประสิทธิภาพในการรักษาหลุมสิวได้ดีกว่าการใช้พลังงานมาตรฐานอย่างมีนัยสำคัญทางสถิติ โดยมีความปลอดภัยสูงและไม่มีผลข้างเคียงรุนแรง
