อัตราการอยู่รอดของรากเทียมในแผนกทันตกรรม โรงพยาบาลกลาง

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาอัตราการอยู่รอดของรากเทียมในผู้มารับบริการ แผนกทันตกรรม โรงพยาบาลกลางวิธีดำเนินการวิจัย: การศึกษานี้เป็นการศึกษาย้อนหลัง ( retrospective cohort study) โดยเก็บข้อมูลจาก เวชระเบียนของผู้ป่วยของแผนกทันตกรรม โรงพยาบาลกลาง ของผู้ป่วยที่ได้รับการรักษาโดยการใส่รากเทียม ตั้งแต่ เดือนมกราคม 2551-ธันวาคม 2561โดยศึกษาถึงลักษณะข้อมูลทั่วไปของผู้ป่วย ข้อมูลของรากเทียม คำนวณอัตราการอยู่รอดของรากเทียมโดยปรับปรุงตามหลักเกณฑ์ของ Albrektsson and colleagues ทำการวิเคราะห์ข้อมูลทางสถิติหาอัตราการอยู่รอดสะสม (cumulative survival rate) โดยใช้การวิเคราะห์ Kaplan-Meier และหาว่า ปัจจัยใดบ้างที่มีผลต่ออัตราการอยู่รอดของรากเทียมโดยใช้ Cox proportional Hazard model พร้อมทั้งนำเสนอค่า Hazard ratio และค่าความเชื่อมั่นร้อยละ 95
ผลการวิจัย: ผลการศึกษาพบว่า รากเทียมทั้งหมด 99 ราก ที่ใส่ในช่วงระยะเวลาดังกล่าว รากเทียมหลุดออก 5 ตัว ( 2 ตัวในคนไข้ที่เสียชีวิต) อัตราการรอดชีพของรากฟันเทียมที่ยังคงอยู่ ในประชากรทั้งหมด คือ 92.06% พบว่า การมีโรคประจำตัว และ การมีโรคปริทันต์ ที่เป็นสาเหตุของการสูญเสียฟัน เป็นปัจจัยที่ส่งผลต่ออัตราการอยู่รอดของรากฟันเทียมอย่างมีนัยสำคัญ พบว่า ผู้ที่มีไขมันในเลือดสูงจะมีความเสี่ยงที่รากฟันเทียมจะหลุดหลังจากการผ่าตัดใส่รากฟันเทียมเท่ากับ 46.9 เท่า ของกลุ่มไม่ทราบว่ามีโรคประจำตัว(unknown) โรคปริทันต์ (periodontitis) จะมีความเสี่ยงที่รากฟันเทียมจะหลุดหลังจากการผ่าตัดใส่รากฟันเทียมเท่ากับ 68.9 เท่า ของกลุ่มที่รากฟันแตก
สรุป: รากเทียมมีอัตราการอยู่รอดที่สูงมาก และการมีโรคประจำตัว โรคปริทันต์เป็นปัจจัยที่เพิ่มความเสี่ยงในการสูญเสียรากฟันเทียม
