บทคัดย่อ
บทนำ: เลือดออกจากทางเดินอาหารส่วนบนในเด็ก เป็นภาวะที่พบได้ตั้งแต่ทารกแรกเกิดจนถึงเด็กโตซึ่งส่วนใหญ่อาการไม่รุนแรง แต่มีบางรายมีอาการเลือดออกรุนแรง ซึ่งต้องได้รับการวินิจฉัยและรักษาอย่างเร่งด่วน จึงมีความสำคัญที่แพทย์ทั่วไป และกุมารแพทย์ ต้องให้การวินิจฉัยสาเหตุ และให้การรักษาเบื้องต้นอย่างเหมาะสม
วัตถุประสงค์: เพื่อทบทวนความรู้เรื่องเลือดออกจากทางเดินอาหารส่วนบนที่ไม่ใช่จากภาวะเส้นเลือดขอดในเด็กเรื่อง สาเหตุ การวินิจฉัย การรักษาด้วยยา และหัตถการห้ามเลือดที่มีการพัฒนาอย่างต่อเนื่องเพื่อเป็นประโยชน์ในการรักษาแก่ผู้ป่วยเด็กที่มีเลือดออกทางเดินอาหารส่วนต้น
วิธีดำเนินการศึกษา: ทบทวนวรรณกรรม
ผลการศึกษา: เลือดออกจากทางเดินอาหารส่วนบนที่ไม่ใช่จากภาวะเส้นเลือดขอดในเด็ก มีสาเหตุแตกต่างกันตามช่วงอายุ สามารถวินิจฉัยได้จากการซักประวัติ ตรวจร่างกาย และควรประเมินสัญญาณชีพเบื้องต้น เพื่อให้การรักษาอย่างเร่งด่วนและเหมาะสม ปัจจุบันมีการรักษาด้วยการให้ยาลดกรด ร่วมกับการส่องกล้องทางเดินอาหารส่วนบนช่วยหาสาเหตุ และสามารถให้การรักษาด้วยหัตถการห้ามเลือดด้วยเทคนิคที่ทันสมัยมากขึ้น ช่วยให้การรักษามีประสิทธิภาพ ลดความเสี่ยงที่ผู้ป่วยต้องได้รับการผ่าตัดเพื่อห้ามเลือด
สรุป: ภาวะเลือดออกจากทางเดินอาหารในเด็กเป็นภาวะเร่งด่วนที่ต้องให้การวินิจฉัยและรักษา โดยเฉพาะในผู้ป่วยที่มีสัญญาณชีพผิดปกติ ปัจจุบันสามารถวินิจฉัยและให้การรักษาห้ามเลือดโดยการส่องกล้องทางเดินอาหารส่วนบน โดยต้องแยกสาเหตุระหว่างเลือดออกจากภาวะที่ไม่ใช่เส้นเลือดขอดหรือเลือดออกจากเส้นเลือดขอด เพื่อให้ยารักษาและทำหัตถการห้ามเลือดอย่างถูกต้องเหมาะสม
