บทคัดย่อ
ศัลยกรรมคืนสภาพเนื้อเยื่อปริทันต์ (Regenerative periodontal surgery) เป็นการผ่าตัดเพื่อแก้ไขรอยโรคกระดูกเบ้าฟันโดยการทำให้เกิดการสร้างใหม่ของอวัยวะปริทันต์ที่เคยสูญเสียจากการเป็นโรคปริทันต์มาก่อน โดยเฉพาะอย่างยิ่งในรอยวิการใต้สันกระดูกแบบเหลือผนัง 2 ด้านและ 3 ด้าน (2-wall and 3-wall intrabony defect) ซึ่งอาจทำได้ด้วยวิธีการชักนำให้เนื้อเยื่อคืนสภาพ (Guided tissue regeneration; GTR) หรือการชักนำให้เนื้อเยื่อคืนสภาพร่วมกับการปลูกถ่ายกระดูก (Guided tissue regeneration with bone graft) เพื่อหวังผลจากคุณสมบัติการสร้างกระดูก (Osteogenesis) การเหนี่ยวนำการสร้างกระดูก (Osteoinduction) หรือการเป็นสื่อนำการสร้างกระดูก (Osteoconduction) ของกระดูกที่ใช้ในการปลูกถ่ายในการเพิ่มผลการคืนสภาพเนื้อเยื่อปริทันต์จากการชักนำให้เนื้อเยื่อคืนสภาพเพียงอย่างเดียว อย่างไรก็ตาม ผลการรักษาด้วยการทำศัลยกรรมคืนสภาพเนื้อเยื่อปริทันต์นั้นมีช่วงกว้าง เนื่องจากการทำศัลยกรรมคืนสภาพเนื้อเยื่อปริทันต์เป็นเทคนิคที่ต้องใช้ความเชี่ยวชาญทางคลินิกและประสบการณ์ของทันตแพทย์ค่อนข้างมาก ดังนั้นจึงต้องเลือกผู้ป่วยและพิจารณาปัจจัยต่างๆ อย่างระมัดระวัง เพื่อให้การรักษาประสบความสำเร็จตามที่คาดหวังไว้ โดยบทความฟื้นฟูวิชาการฉบับนี้จะแสดงให้เห็นแนวทางในการเลือกทำศัลยกรรมคืนสภาพเนื้อเยื่อปริทันต์ ระหว่างการชักนำให้เนื้อเยื่อคืนสภาพ หรือการชักนำให้เนื้อเยื่อคืนสภาพร่วมกับการปลูกถ่ายกระดูก อย่างเป็นระบบ ตั้งแต่ขั้นตอนการเลือกผู้ป่วย ขั้นตอนการประเมินรอยโรค และขั้นตอนการเลือกวิธีการรักษา ซึ่งจะทำให้ทันตแพทย์นำไปใช้วางแผนการรักษาให้ผู้ป่วยได้อย่างแม่นยำ
