Pregnancy outcomes in adolescent women at Surin Hospital

Abstract
Background: Pregnancy in adolescence have been considered as a high risk situation that increased adverse obstetric and perinatal outcomes when compare with normal age pregnancy. But some studies failed to confirm this association. The study aimed to determine
Objective: the incidence of adolescent pregnant women and compare obstetric and perinatal outcomes between adolescent and adult pregnant women.
Setting: Department of Obstetrics and Gynecology. Surin hospital, Thailand.
Study design: Retrospective control study.
Method: The study performed analyses of 19,985 pregnant women 10-19 (study group) and 20-34 years old (control group) who delivered during October 1st, 2006 to September 30th , 2010. The data was retrieved from electronic delivery center program included characteristic, obstetric, neonatal outcomes and analysed by SPSS program using relative risk with 95% confidence interval. The study group and control group consisted of 3,176 and 16,809 woman respectively. This incidence of adolescent pregnant women was 13.82% (in all age group) The study group had inadequate antenatal care than control group (41.1% VS 23.3%, p = 0.001)
Results: The study group was associated with anemia (RR = 0.65 ; 95%CI = 0.61-0.71) preterm labor (RR = 0.49;95%CI = 0.44-0.55) normal delivery (RR = 0.44 ; 95%CI = 0.40-0.47) low birth weight (RR = 0.47 ; 95%CI = 0.43-0.52) Apgar score at 1 min 7 (RR = 0.75 ; 95%CI = 0.62-0.90) Apgar score at 5 min 7 (RR = 0.72 ; 95%CI = 0.55-0.95) but lower risk of cesarean section (RR = 2.07 ; 95%CI = 1.90-2.26) operative vaginal delivery (RR = 1.83 ; 95%CI = 1.57-2.13).
Conclusion: The incidence of adolescent pregnant woman was 13.82%. This group had higher incidence of inadequate antenatal care and increased the risk of adverse obstetric and perinatal outcomes in anemia, preterm labor, low birth weight and low Apgar score.
