Diagnostic Accuracy of Modified Alvarado Score in the Diagnosis of Acute Appendicitisat Lahansai Hospital Burirum

Abstract
Background: Acute appendicitis is a common surgical condition with lifetime prevalence of 7-8%, prompt diagnosis is elusive in order to reduce morbidity and to avoid serious complication.
Objective: To determine the diagnostic accuracy of Modified Alvarado score in the diagnosis of acute appendicitis.
Research design: Descriptive retrospective study
Patients: All of 123 consecutive purposive sampling patients who was diagnosed acute appendicitis and underwent appendectomy in Lahansai Hospital.
Methods: Data was collected from medical record of 123 patients who underwent appendectomy during 1st. October 2009 to 30th. June 2010. Research tool was a collective form of Modify Alvarado Score. They were given specific scores according to the variable and divided into two groups, group one (score >7) group two (score <7). Postoperative, diagnosis was confirmed by routinely histopathology report. Validity of scoring system was assessed by calculating accuracy, sensitivity, specificity and positive predictive value.
Results: Total 123 patients were included in the study, which included 46 males and 77 females, at score >7, appendicitis was confirmed in 113/114 patients, while at scores <7 appendicitis was confirmed in 5/9 patients. The accuracy was 92.68 %, the sensitivity was 95.76 %, specificity was 80 % and positive predictive value was 99.12 %.
Conclusion: This scoring system is easy, accuracy, effective and effectiveness aid for
supporting the diagnosis of acute appendicitis.
