Pediatric Urinary Ttract Infection in Surin Hospital

Abstract

Objective: To study organisms that caused urinary tract infection in pediatrics, their sensitivity to antibiotics and pathology of pediatric urinary tract infection patient.
Setting: Pediatric department, Surin hospital
Study design: Descriptive retrospective study
Method: A retrospective review of pediatric patient’s medical record admitted with urinary tract infection during 1st October 2005 to 30th September 2008 was collected.
Result: From overall 144 patients; most of them (55 cases) aged less than one year. Male to female ratio categorized by age 0-1 year, over 1-2 years, over 2-5 years and over 5-15 years were 1.04:1, 1.13:1, 1:2.6 and 1:1.17 respectively. The most common organism was E. coli found in 117 cases (82.06%) followed by Klebsiella found in 16 cases (11.11%). Sensitivity of E. coli to Cefotaxime and Ceftazidime were 88.8% and 91.3%, while sensitivity to Gentamicin, Amikacin, Co-amoxiclav, Co-trimoxazole and Ampicillin were 68.8%, 64.5%, 45.8%, 31.8% and 14.6% respectively. Kidney Ultrasonography and Voiding Cystourethrography were completely done in only 54 cases (37.50%) and abnormalities were found in 17 cases (11.81%) in which 16 cases of these were Vesicoureteral reflux.
Conclusion: E. coli is the most common cause of pediatric urinary tract infection. Sensitivity of E. coli to 3rd generation Cephalosporin is quite high while sensitivity to Aminoglycoside is moderate and to Co-amoxiclav, Co-trimoxazole and Ampicillin is low. Vesicoureteral reflux is the most often abnormality of urinary tract system found after investigation.
