Abstract

Objective: To study outcome of Primary resection and anastomosis after bowel decompression in patient diagnosed as left side colonic obstruction in Sisaket hospital between 2002-2005
Study design: Descriptive, Retrospective study
Patient and Method: All patients admitted to Sisaket Hospital from 2002-2005 with left side colonic obstruction and operated with single stage resection and anastomosis without intraoperative colonic lavage were reviewed about operating time, postoperative hospital stay and infection rate
Result: A total of 23 patients were operated by resection and primary colonic anastomosis without intraoperative colonic lavage.Every patients had evidenced of colonic obstruction and large bowel was decompressed by manual decompression. Nobody developed anastomotic leakage 5death had not occurred postoperatively, only one patient who was diagnosed as advanced CA colon at splenic flexure and had Enbloc resection(left hemicolectomy + distalpancreatectomy + splenectomy) had wound infection.The mean operating time was 116 min. Post operative recovery period in hospital was 9.48 days.
Conclusion: Manaul decompression instead of intraoperative colonic lavage insingle stage resection and primary anastomosis may be safe procedure in management left side colonic obstruction.
