Evaluation of Acute Appendicitis Guideline of Surin Hospital

Abstract

Background: CPG is nearly well established as an routine as guideline for the majority of clinical practice in both urban and rural hospitals. Surprisingly, very few of them are verified in term of their application in the real situation, especially in rural hospitals, which have different resources and background from the urban hospital.
Objective: To evaluate the results of treatment of acute appendicitis before and after using CPG Acute appendicitis
Design: Action Research
Setting: Department of surgery, Surin Hospital
Patient and Methods: 156 Patients were diagnosed acute appendicitis, 46 cases were in the first period, before using CPG, during 1-31 October 2001 and 110 cases were in the second period during 1 December 2002 to 28 February 2003 after using CPG Acute Appendicitis which was implemented in surgical department of Surin Hospital. The waiting time to operation, Length of stay ; cost of treatment and post operative complication were compared between the two groups
Results: After using CPG, mean waiting time to operation was 5.3 + 5.7 hours in the first period and 2.06 + 2.15 hours in the second period (P < 0.001) length of stay was 4.1 + 1.8 days, 3.31+ 0.9 days in respectively the first period and the second period. (PcO.001). The cost of Antibiotic drugs and the post operative complication were decreased after using the CPG
Conclusion: CPG can apply and use to improve quality of treatment.
