Nursing Care In Diabetic with Complication In Buriram Hospital (Case study)

Abstract

Introduction: Increasing in number of diabetic patient hospitalized with its complication in female medicine ward, Buriram hospital interested nurse profession to study nursing care for these patients.
Objective: To create nursing care guidelines and discharge plan for diabetic patient with complication to promote self care for patients and their families
Study design: Descriptive Study (Two cases of diabetic patients with complication admitted at medicine ward of Buriram Hospital during July to September 2007)
Result: The first case was 66 year’s old Thai woman diagnosed with hypoglycemia (underlying diabetes and hypertension). Nursing care plan includes blood sugar and vital signs monitoring, assessed patient consciousness, control fluid rate, observed symptoms of hypoglycemia, electrolyte imbalance and adverse drug event including prevent risk of fall. Discharge plan comprise of medication adherence, diet control and continuum of care. Patient was recovered and discharged after two days of admission. Home care visit found that patient has an appropriated life-style modification.
The second case was 39 year’s old Thai woman diagnosed with acute pyelonephritis (underlying diabetes and hypertension). Nursing care plan includes blood sugar and vital signs monitoring (every 4 hour), Antibiotic and Insulin administration, observed symptoms of Aminoglycosides adverse event and septic shock. After 13 days, patient was recovered and discharged. Patient was referred to home health care program according to problems of drug non-compliance, personal hygiene, voiding suppression, herbal medicine misuse and occupational problem. After home visit, patient had better self care management.
Conclusion: Comprehensive knowledge in pathophysiology, treatment plan, medication and its complication, patient’s data collection and analysis for holistic nursing care plan. These will benefit for patients to prevent disease crisis and recurrence of complication by life-style modification.
