Cerebellar Involvement of Scrub Typhus in Children at Sisaket Hospital

Abstract
Objective: To determine the incidence of scrub typhus, clinical manifestation, laboratory finding and report a case of acute cerebellar ataxia as neurologic complication. Patients and method: Case records of scrub typhus from January 1999 to December 2000 were retrospectively reviewed. The diagnosis was based on clinical manifestation, serologic test and therapeutic response.
Results: A total of 23 patients were examined during studied period. There were 19 males and 4 females with the age average of 9 year - old. Scrub typhus is a rural disease. The high prevalence of disease occured during rainy season. The clinical manifestations included prolonged fever and headache which where major symptoms. Where as eschar was founded only 30%. The hepatosplenomegaly lymphadenopathy was 13%. Although focal neurological signs were rare, but in this study two cases of acute cerebellar ataxia were founded. The symptoms started during the second week of fever including truncal ataxia, nystagmus, intention tremor. Thrombocytopenia was founded in 4 patients (17 %), therefore scrub typhus must have been in differential diagnosis of Dengue hemorrhagic fever.
Conclusions: Scrub typhus is a tropical rural disease which occured mainly during rainy season and found in male greater than in female. Lacking the classical textbook presentations, scrub typhus often missed. Recognition is important because scrub typhus respond well to doxycycline.
