Prognostic Factors of Late ST Elevation Myocardial Infarction in Buri Ram Hospital

Abstract

Background: ST elevation myocardial infarction (STEMI) is the leading cause of death in the genera population. Late STEMI (diagnosis time ≥12 hours after onset of chest pain) causes higher morbidity and mortality rate. Modifiable factors may reduce late STEMI and improved outcome.
Objective: To study the prognostic factors of late STEMI in Buri Ram hospital.
Methods: This was a retrospective observational cohort study in patient who admitted with STEMI in Buri Ram hospital from 1st January 2017 to 31st October 2020. We analyzed risk factors of late STEMI by multivariable risk difference regression.
Results: Fifty-five patients were in late STEMI (≥12 hours) group and 564 patients were in early STEMI group.The significant factors for late STEMI group were late visit (adjusted risk difference 72.8%), misdiagnosis (adjusted risk difference 71.4%). Factors related to sex, age, diabetes, ischemic heart disease were not foundstatistically significant.
Conclusion: In this study, late visit and misdiagnosis were the important problems for late STEMI and may increase in-hospital mortality in Buri Ram hospital.
