Comparison of Digit Pain Score Between Plastic and Stainless-steel Finger-trap Traction in Closed Reduction in Distal End Radius Fracture, Randomized Controlled trial

Abstract
Purpose: The Chinese finger trap is a device used to aid in the reduction of fractures, especially in the distal end of radius fracture. The stainless-steel finger trap is widely used but often cause finger pain and expensive. We used a plastic finger trap to reduce pain and provide a lower cost alternative.
Study design: Randomized Control Trial
Methods: We prospective enrolled consecutive 26 patients between June 2023 and December 2023 who underwent closed reduction and immobilized with a short arm cast in Sangkha hospital. All patients were randomly assigned to stainless-steel finger trap using group and plastic finger trap using group. For each trial, patients lay supine on the bed, and the finger trap was attached to thumb, index and middle finger of patients, with the upper arm suspended by a plastic sheet loop. Weights were added by weight bag that suspended over the patient’s upper arm. From the 0th to 5th minutes, we used two kilograms of weight bag. From the 6th to 15th minutes, we used three kilograms of weight bag. Patients were completed visual analog scale (VAS) pain scoring questionnaires during finger traction with Chinese finger trap. After 24 hours after short arm casting, all patients will be asked for complications.
Results: From the use of finger traction devices, both plastic and stainless-steel, in 26 patients with closed distal end radius fractures, it was found that the group using plastic finger traction devices experienced less pain (mean 2.09, SD 1.66) compared to the group using stainless-steel devices (mean 4.01, SD 1.75) between minute 6 to 14. The use of plastic finger traps significantly reduces finger pain, with statistical significance (p-value <0.05). There were no reported complications in the plastic group. On the other hand, the stainless-steel group showed superficial skin injury around finger 24 hours after using the finger traction device in 2 patients.
Conclusion: The plastic finger traction device proves to be an effective alternative compared to the stainless-steel finger traction device, resulting in less pain
