Development of a Change Management Model to Enhance an Excellence Health Service for Subdistrict Health Promoting Hospital transferred to Saraburi Provincial Administrative Organization

Abstract

Background: After the transfer of the Subdistrict Health Promoting Hospital to the local administrative organization Found a problem of unpreparedness in management. As a result, personnel were confused and lack morale and affect the quality and continuity of health services. In fiscal year 2023, Saraburi Province transferred the 30 Subdistrict health promotion hospitals (23.8%) and coverage will be 100 percent in fiscal year 2025. Therefore, proper preparation will prevent problems and reduce risks arising from the effects of the change.
Objective: To study the needs and development opportunities of the subdistrict health promoting hospital and to develop of a change management model to enhance an excellence health service for subdistrict health promoting hospital transferred to Saraburi provincial administrative organization
Methods: This research was a mixed methods method. The sample group consists of: the executive of the public health agencies and the local organizations, the experts, the personals working in subdistrict health promoting hospital. And the Health networks included 220 samples, selected by purposive, simple random, and convenience random sampling, The collected data by interviews, questionnaires, and focus groups discussion. Data were analyzed by descriptive statistics, Inferential statistics, and content analysis.
Results: 1) The development needs of the Subdistrict Health Promoting Hospital were training and developing skills and knowledge in local regulations, Improvement service facilities and medical equipment, Management financial resources effectively, creating policy health base on with local needs, using technology and communications to promote information exchange, and Development of innovative health services. 2) The change management model to enhance an excellence health service for subdistrict health promoting hospital transferred to Saraburi provincial administrative organization is the new knowledge for management to enhance the quality of health services and the competency of health personal of Subdistrict Health Promotion Hospitals transferred to Provincial Administrative Organizations. The development process consists of 9 process; 1) Creation of committees and transfer work of Subdistrict Health Promoting Hospitals., 2) Analyzing needs, 3) Determinate of development goals,4) Creating of the Memorandum of Understanding, 5) Communication of the Health network cooperation, 6) Creating culture of the change learning organizational, 7) Strengthening of the standards and sustainable on the local health service, 8) Creating of the local public area for the public health, and 9) Monitoring and evaluation. The model was considered high level in actually utility ( =3.9, S.D.=0.77) and feasibility ( =3.8, S.D.=0.63) could be applied to Enhance an Excellence Health Service for Subdistrict Health Promoting Hospital transferred to Saraburi Provincial Administrative Organization.
Conclusions: The change management model to enhance an excellence health service for subdistrict health promoting hospital transferred to Saraburi provincial administrative organization were 9 processes. Saraburi Provincial Administrative Organization and Saraburi Provincial Public Health Office should be using the change management model to be a guideline of prepare and promote potential for regarding the quality of local public health services of Subdistrict Health Promotion Hospital, and supporting to development on personals, organizations, and resources that corresponds to the needs of the Subdistrict Health Promoting Hospital.
