Endodontics management of external root resorption following replantation of an avulsed permanent tooth : A case report

Abstract

Background: Dental trauma with avulsion of permanent teeth occurs in 0.5–3% of all maxillofacial trauma patient and most commonly involves the maxillary anterior teeth. It is most frequently seen in children aged 8–12 years, with a 2.5-fold higher incidence in boys than girls. The key factor in the management of an avulsed tooth is maintaining the viability of the periodontal ligament cells on the root surface, as this directly influences the risk of root resorption or subsequent ankylosis, both of which can complicate long-term treatment.
Objective: To present a method of replantation for avulsed permanent tooth with closed apex and extra oral dry time more than 60 minites.
Methods: A case study was conducted through a retrospective review of the patient’s medical and dental records. Data collected included presenting symptoms, clinical and radiographic findings, treatment modalities and follow-up outcomes.
Results: The results from this study showed that the root resorption was first detected month 2 and began to stabilized by month 6. At the 3 month followed up, the treated tooth was clinically asymptomatic with normal mobility and normal percussion response and showed no sign of ankylosis. Radiographic evaluation revealed evidence of replacement resorption with an intact lamina dura.
Conclusions: A tooth that has been avulsed due to trauma should be replanted as soon as possible. Proper and timely management is crucial for achieving a favorable prognosis. Regular follow-up appointments are essential to detect potential complications, plan appropriate treatment in accordance with the patient’s jaw growth and development, and consider both functional and esthetic outcomes.
