The Effect of Dementia Screening on Glycemic Control among Older Adults with Type 2 Diabetes at Prasat Hospital, Surin Province

Abstract
Background: Older adults with type 2 diabetes mellitus who also have dementia may experience difficulties in disease control. Therefore, dementia screening plays an important role in comprehensive diabetes care and may contribute to improved glycemic control.
Objective: To evaluate the effect of dementia screening on glycemic control, measured by glycated hemoglobin (HbA1c), among older adults with type 2 diabetes mellitus at Prasat Hospital, Surin Province.
Methods: A retrospective cohort study was conducted among older adults with type 2 diabetes mellitus. A total of 84 participants were included and categorized into a dementia screening group and a control group, comprising 28 and 56 participants, respectively. Data were analyzed using a Linear Mixed-Effects Model (LMM).
Results: Most participants were female. The screened group had a higher median age than the control group [70 (66.3–78.3) vs 67 (63.0–74.8) years; p < 0.05]. Abnormal TMSE screening results (<24 points) were found in 67.9% of participants, while other baseline characteristics were not significantly different between the groups. The study also found no statistically significant interaction effect between dementia screening, group, and time on glycemic control as measured by glycated hemoglobin (HbA1c) levels (F(1,82)=1.359, p=0.247).
Conclusions: Dementia screening in older adults with type 2 diabetes mellitus was not significantly associated with glycemic control as measured by HbA1c levels. The major factor associated with good glycemic control was baseline HbA1c. Therefore, older adults with poorly controlled diabetes should receive continuous follow-up for appropriate assessment and treatment adjustment.
