Transcutaneous Bilirubin Measurement in Full Term Neonate by Digital Camera

Abstract

Background: Neonatal jaundice remains one of the most common symptoms in newborn. Total serum bilirubin (TSB) level assessment in clinical laboratory is an objective method. It is painful and possibility of local infection. Transcutaneous measurement of bilirubin concentration is considered to be ideal for neonatal jaundice screening. The authors develop the new method prototype for measures serum bilirubin transcutaneous by digital camera.
Objective: To design the new noninvasive method for measure transcutaneous bilirubin level of the newborn by digital camera.
Materials and methods: The new noninvasive method use digital camera and control paper. Take photograph the newborn and measurement by use L*a*b system measure at sternum chest wall at lightness = 70%, measurement value of Y and M by CMYK system, use M subtract from Y to get the real value of yellow of bilirubin. The correlation between total serum bilirubin and value of Y-M was assessed by Pearson’s product moment correlation and linear regression. The total trancutaneous digital bilirubin (TcdB) can calculate by TcdB= 0.264 + 0.58x(Y-M)+2.9 mg%
Results: The study group consisted of 61 newborns (31 males, 30 female). A series of linear relationships were found to exist between total serum bilirubin concentration and value of Y-M, The mean total serum bilirubin = 12.55+4.2 mg% and mean total digital transcutaneous = 12.92+3.4 mg%, coefficient of correlation = 0.86, coefficient of determinates (r2)=77.4, standard error of estimation = 1.38 .Significant statistic between value of serum bilirubin and value of Y-M was found (P < 0.05)
Conclusions: We conclude that the new digital transcutaneous bilirubin measurement method shows a good correlation with standard laboratory method. It will become alternative to the routine procedure used to measure serum bilirubin concentration in the near future.
