Factors Relating to Locally Advanced Breast Cancer in Rural Areas.

Abstract
Background: Locally advanced breast cancer (LABC) is high incidence in rural areas especially in northeast part of Thailand. LABC remains a major clinical problem and the prognosis is poor.
Objective: The goals of this study needed to find the factors that relating to LABC and plan for created a proper strategy to reduce the incidence of LABC.
Methods: A study design was a prospective cohort observational study conducted at Surin hospital from January 2020 to January 2022. New cases of breast cancer patients were approached and interviewed before treatment, observed during treatment and followed 1 year after diagnosed.
Results: 141 patients were diagnosed with breast cancer and 20 patients were excluded. A total of 121 patients were approached, the mean age at diagnosed was 52.9 years. At presentation 46.3 % (95%CI 37.2%, 55.6%) of breast cancer patients were locally advanced stage. The factors relating to LABC were duration of symptoms before treatment (p < 0.001), nulliparity (p = 0.037), tumor grade (p = 0.026) and presence of angiolymphatic invasion (p = 0.022). 1- year survival in LABC is less than early breast cancer significantly (82.1% vs 100%, p < 0.001).
Conclusion: Prolonged duration of symptoms before treatment, nulliparity, high tumor grade and presence of angiolymphatic invasion were factors relating to locally advanced breast cancer in rural areas. The proper strategy to reduced duration of symptoms before treatment may decrease the incidence of LABC in rural areas.
