Abstract

Management of furcation involvement in severe periodontitis is a challenging problem, particularly in mandibular molars with Class II involvement. Regenerative periodontal surgery has been reported to improve tooth prognosis and reduce the risk of tooth loss. In the present case report, clinical and radiographic findings revealed a mandibular right second molar with a Class II furcation defect and severe interradicular bone loss as shown radiographically. The tooth was treated with periodontal surgery using freeze-dried bone allograft without membrane placement. At the 18-month follow-up, clinical evaluation revealed a reduction in probing depth and improvement in clinical attachment level. Radiographic analysis showed an approximate 50% increase in bone height and density at the furcation site, with no periapical pathology. The patient reported transient functional hypersensitivity during mastication at 15 months, which completely resolved by 18 months. The tooth remained vital and, responding positively to electrical pulp testing. This case suggests that bone grafting alone may be a viable treatment option in selected cases, particularly in patients with limited resources. It also highlights the importance of long-term follow-up to evaluate the stability of periodontal tissues.
