บทคัดย่อ
หลักการและเหตุผล: การส่องกล้องโพรงมดลูก (Hysteroscopy) ปัจจุบันเป็นการส่งตรวจวินิจฉัยพยาธิ สภาพในโพรงมดลูก ที่ได้ผลการตรวจเป็นที่น่าเชื่อถือมากกว่าการส่งตรวจอื่น โดยการ ส่องกล้องโพรงมดลูกแบ่งเป็นสองวิธีตามเครื่องมือที่ใช้ในการส่องกล้อง ได้แก่ Rigid hysteroscopy (กล้องแบบแข็ง) และ Flexible hysteroscopy (กล้องแบบอ่อน) โรงพยาบาลมหาราชนครราชสีมาได้เริ่มมีการนำเครื่องมือส่องกล้องโพรงมดลูกมาใช้ ตั้งแต่เมษายน พ.ศ. 2553 โดยทำการส่องกล้องแบบอ่อนในการตรวจวินิจฉัยเบื้องต้น แบบผู้ป่วยนอก ในระยะ 4 ปีที่ผ่านมาได้มีการทำการส่องกล้องโพรงมดลูกไปมากกว่า 300 รายการศึกษาเพื่อทราบถึงข้อมูลเบื้องต้นทั้งประสิทธิผลและภาวะแทรกซ้อน ของการส่องกล้องโพรงมดลูก สามารถใช้เป็นแนวทางในการพัฒนาการดูแลผู้ป่วยและเทคนิคการส่องกล้องในโพรงมดลูกต่อไป
วัตถุประสงค์: เพื่อศึกษาประสิทธิผลและภาวะแทรกซ้อนของการส่องกล้องโพรงมดลูกแบบผู้ป่วนอกในโรงพยาบาลมหาราชนครราชสีมา
สถานที่ศึกษา: กลุ่มงานสูติ-นรีเวชกรรม โรงพยาบาลมหาราชนครราชสีมา
รูปแบบการศึกษา: การศึกษาแบบ retrospective hospital-based descriptive study
วิธีการศึกษา: โดยรวบรวมรายงานย้อนหลัง จากข้อมูลบันทึกการส่องกล้องโพรงมดลูกจากเวชระเบียน โรงพยาบาลของสตรีทุกรายที่มาทำการส่องกล้องโพรงมดลูกแบบผู้ป่วยนอกที่โรงพยาบาลมหาราชนครราชสีมา ระหว่าง เมษายน พ.ศ. 2553 - มีนาคม พ.ศ. 2557
ผลการศึกษา: พบสตรีที่เข้ารับการส่องกล้องโพรงมดลูกจำนวน 379 ราย โดยข้อบ่งชี้ที่มาทำการ ส่องกล้องโพรงมดลูกมากที่สุดได้แก่ พบความผิดปกติจากอัลตร้าซาวน์ ภาวะเลือดออก ผิดปกติ และภาวะมีบุตรยาก ตามลำดับ ผลการส่องกล้องโพรงมดลูกพบความผิดปกติ จำนวน 230 ราย ร้อยละ 60.7 ของสตรีที่มาทำการส่องกล้องทั้งหมด พบไม่สามารถ ส่องกล้องได้สำเร็จจำนวน 28 ราย ร้อยละ 7.4 โดยสาเหตุที่ส่องกล้องโพรงมดลูก ไม่สำเร็จมากที่สุดได้แก่ ปากมดลูกตีบ ภาวะความเจ็บปวด และการให้ภาพจากการส่องกล้องไม่สมบูรณ์ ตามลำดับ พบภาวะแทรกซ้อนปากมดลูกฉีกขาด 2 รายและ ไม่พบภาวะแทรกซ้อนที่รุนแรงจากการส่องกล้องโพรงมดลูก
สรุป: การส่องกล้องโพรงมดลูกแบบผู้ป่วยนอกสามารถทำได้อย่างมีประสิทธิผลและ มีความปลอดภัยสูง
