บทคัดย่อ
ที่มา: โรค Retinopathy of Prematurity (ROP) เป็นภาวะแทรกซ้อนที่รุนแรง และ พบได้บ่อยในทารกคลอดก่อนกำหนด ซึ่งเป็นสาเหตุทำให้เด็กตาบอดได้และนำมาซึ่งปัญหาการฟ้องร้องในปัจจุบัน อันเนื่องมาจากความไม่เข้าใจในโรคของบิดา มารดาของทารก
วัตถุประสงค์: เพื่อศึกษาอุบัติการณ์ของการเกิด ROP และอัตราการรักษาโรค ROP ในทารกคลอดก่อนกำหนดในโรงพยาบาลบุรีรัมย์
รูปแบบการศึกษา: การศึกษาย้อนหลัง แบบพรรณนา
กลุ่มเป้าหมาย: ทารกคลอดก่อนกำหนดที่เสี่ยงต่อการเกิด ROP จำนวน 203 ราย ที่ได้รับการตรวจ คัดกรองหา ROP ระหว่างวันที่ 1 มกราคม พ.ศ. 2549 ถึง 31 ธันวาคม พ.ศ. 2549
วิธีการศึกษา: รวบรวมข้อมูลผู้ป่วยจากการบันทึกทางการแพทย์และวิเคราะห์
ผลการศึกษา: อุบัติการณ์การเกิด ROP ในโรงพยาบาลบุรีรัมย์ ปี พ.ศ. 2549 พบ 21.18% (43/203) และพบ ROP ตั้งแต่ stage 3 ขึ้นไป 8.37% (17/203) และมีอุบัติการณ์ของผู้ป่วย ROP ที่ต้องได้รับการรักษา 3.94% (8/203) ในกลุ่มทารก ที่เกิด ROP พบค่าเฉลี่ยของอายุครรภ์ Gestational age เท่ากับ 30.16 สัปดาห์ และพบค่าน้ำหนักแรกคลอดเฉลี่ยเท่ากับ 1,330 กรัม เมื่อทารกได้รับการตรวจ ตาครั้งแรก ค่าเฉลี่ยของอายุทารกหลังคลอด Chronological age และค่าเฉลี่ย ของ Postmenstrual age (อายุครรภรวมกับอายุทารกหลังคลอด) เท่ากับ 39.37 วัน และ 35.69 สัปดาห์ ตามลำดับ เมื่อตรวจพบ ROP ครั้งแรก อายุ ทารกหลังคลอด Chronological age เฉลี่ยเท่ากับ 42.44 วัน และค่าเฉลี่ย ของ Postmenstrual age เท่ากับ 36.12 สัปดาห์ พบอายุทารกหลังคลอด Chronological age เฉลี่ยและ Postmenstrual age เฉลี่ย ในวันที่ตรวจพบ ROP stage 3 เท่ากับ 52.53 วัน และ 36.94 สัปดาห์ ตามลำดับ พบอายุทารก หลังคลอด Chronological age เฉลี่ย และ Postmenstrual age เฉลี่ยในวันที่ ทำการรักษา เท่ากัน 57.38 วัน และ 36.88 สัปดาห์ ตามลำดับ
สรุป: จากการศึกษานี้พบว่าอุบัติการณ์ของการเกิด ROP ในโรงพยาบาลบุรีรัมย์ ปี พ.ศ. 2549 เท่ากับ 21.18 % และมีอุบัติการณ์ของผู้ป่วย ROP ที่ต้องให้การ รักษาจำนวน 3.94 % และพบว่าทารกที่มีอายุครรภ์น้อยกว่า 34 สัปดาห์ หรือ ทารกที่มีน้ำหนักแรกคลอดน้อยกว่า 1,500 กรัม เสี่ยงต่อการเกิด Threshold ROP
