บทคัดย่อ
หลักการและเหตุผล: โรงพยาบาลแก้งคร้อเริ่มให้บริการส่องกล้องตรวจทางเดินอาหารส่วนต้นเมื่อวันที่ 15 กันยายน พ.ศ. 2560 การศึกษานี้รวบรวมผลลัพธ์ของบริการส่องกล้อง เพื่อทราบถึงวินิจฉัยโรคข้อมูลการติดเชื้อเฮลิโคแบคเตอร์ไพโลไรและอุบัติการณ์มะเร็งทางเดินอาหารส่วนต้นในพื้นที่
วิธีการศึกษา: ศึกษาแบบ Descriptive retrospective study เก็บข้อมูลผู้ป่วยที่เข้ารับการส่องกล้อง 358 ราย ระหว่าง 15 กันยายน พ.ศ.2560 ถึง 30 กันยายน พ.ศ.2563 จากการสัมภาษณ์ และฐานข้อมูลโรงพยาบาลแก้งคร้อ
ผลการศึกษา: โรคที่พบมากที่สุดในพื้นที่คือการติดเชื้อเฮลิโคแบคเตอร์ไพโลไร 123 ราย (ร้อยละ 34) พบร่วมกับกระเพาะอาหารอักเสบร้อยละ 67 การติดเชื้อเพิ่มการเกิดกระเพาะอาหารหรือลำไส้ดูโอดีนัมอักเสบมากกว่าผู้ป่วยที่ไม่ติดเชื้ออย่างมีนัยสำคัญ (p<0.001) และสัมพันธ์กับผลตรวจพยาธิวิทยาผิดปกติ คือ กระเพาะอาหารอักเสบรุนแรง กระเพาะอาหารอักเสบเลือดออกและเยื่อบุกระเพาะอาหารเปลี่ยนเป็นเยื่อบุลำไส้อย่างมีนัยสำคัญอันดับสองคือFunctional dyspepsia 89 ราย (ร้อยละ 24) และอันดับสามกระเพาะอาหารอักเสบ 47 ราย (ร้อยละ 13) อุบัติการณ์มะเร็งหลอดอาหาร 2 ราย (2.6:100,000 ประชากร) อุบัติการณ์มะเร็งกระเพาะอาหาร 5 ราย (6.6:100,000 ประชากร) อุบัติการณ์มะเร็งทั้ง 2 ชนิด น้อยกว่าประชากรไทยทั่วไปแต่การศึกษานี้จำนวนประชากรน้อย ไม่พบผู้ป่วยมะเร็งลำไส้ดูโอดีนัมในพื้นที่เนื่องจากเป็นมะเร็งที่มีอุบัติการณ์ต่ำ
สรุป: โรคที่พบมากที่สุดในพื้นที่คือการติดเชื้อเฮลิโคแบคเตอร์ไพโลไร Functional dyspepsia และกระเพาะอาหารอักเสบ อุบัติการณ์มะเร็งหลอดอาหาร 2.6:100,000 ประชากร อุบัติการณ์มะเร็งกระเพาะอาหาร 6.6:100,000 ประชากร
