Development of Nursing Care System for End Stage Renal Disease Patient with Continuous Ambulatory Peritoneal Dialysis (CAPD)

Abstract
Background: End-stage renal disease is one of important current health problem which continuously increases and affect various organ’s function. Continuous ambulatory peritoneal dialysis (CAPD) is an important intervention in which increases patient’s accessibility to the treatment and decrease morbidity.
Objective: to study mortality rate, infectious complication and satisfaction of patient and nurse practitioner to the developed CAPD nursing care system.
Study Design: Action Research
Methods: the study was comprised of three phases; phase 1) situation analysis and relevant factors identification, phase 2) nursing care process development and phase 3) application the developed process to practice. The process was developed following plan do check improvement and assessment cycle. This study was conducted from October 2009 to September 2014. Research tool was quality of care assessment form. Data was collected according to quality of care indicators in end-stage renal failure patient undergoing peritoneal dialysis such as mortality rate and peritoneal infection rate. The indicators data was collected during 2009-2014 in 114, 160, 201 and 259 patients respectively. Patient and nurse satisfaction questionnaire was approved and adjusted for content validity by expert opinion before applied to 30 patients and 160 registered nurses.
Results: After the developed nursing care system was applied to practice in 2015, result found that mortality rate decreased from 15.4 to 10.0 and catheter exit site or tunnel related infection decrease from 45.8 to 19.7. Patient and nurse had significantly increase satisfaction score (p<0.001).
Conclusion: The developed nursing care system resulted in decrease mortality and infectious complication furthermore increase patient and nurse satisfaction, end stage renal disease, continuous
