Outcome of Easy Asthma Clinic Krasang Hospital Buriram Province

Abstract
Background: Asthma is common chronic disease and acute asthmatic attack is one of leading cause of emergency visit and hospitalization. According to the survey, the asthma treatment in Thailand was poorly managed. From above rationale, Krasang hospital has established the Easy asthma clinic based on the asthma management guideline issued by department of medicine Khonkean University since October 2005 which applied from the GINA guideline for total asthma control goal.
Objective: to analyze and compare level of asthma control, emergency visit, hospitalization rate and complications of the patient at pre and post registration in Easy asthma clinic.
Study design: Retrospective study
Methods: The studied population is asthmatic patients aged > 5 years old who attended the asthma clinic for at least 36 months. 105 patients were included in this study. Research tool includes asthma clinic medical records from 1 October 2005 to 30 September 2011 which evaluated the patients after management at 0, 12, 24 and 36 months. Descriptive statistic was performed and reported in term percent mean and standard deviation. Analytical statistic was also performed using analysis of variance (ANOVA), Chi-square, Paired t-test and Fisher exact test.
Result: Clinical outcome measured by level of asthma control was done before and after management at 0, 12, 24 and 36 months, significant improvement has been shown (p =0.000). The patients achieved asthma control from 14.1% to 59.0%, 57.3% and 53.3% respectively. The comparison of emergency visit and hospitalization rate revealed statistically significant improvement (p =0.000). Frequency of emergency visit decreased from 74.8% to 10.5%, 4.8% and 4%. Hospitalization decreased from 40% to 2.9%, 1.9% and 5%. This study also found that dose of inhale steroid was not correlated with TB and pneumonia complications.
Conclusion: This study suggested the implementation of Easy asthma clinic in community hospital will allows asthmatic patient to access standard treatment to improve better asthma control and patient safety.
