Health Status and Factors Affected Mortality of Stroke Patients After Discharged from Surin Hospital

Abstract
Background: According to Surin hospital data in 2003, Stroke is the third of mortality cause after heart disease and accident and intoxication. These resulted from stroke patients who admitted in Surin hospital received rehabilitation in short period and difficulty to follow up.
Purpose: To describe patient’s health status after discharged from Surin hospital and identify factors
related to mortality of stroke patients.
Method: The study was conducted in stroke patients of Surin hospital. By specific randomization, 298 patients were included in this study. The content of questionnaire for assess capability of stroke patient had two sections; the first four topics pertained functioning of paraplegia/ hemiplegia parts and last 13 topics pertained self-performance in daily life activities. Analytical study of health status aims to verify relationship between one year mortality after discharged from Surin hospital and chronic diseases such as diabetes, hypertension and heart disease. Multiple logistic regression was used at significant level oc = 0.05.
Result: Demographic data of stroke patients were male 36.58% and female 63.42%, aged between 71-80 years (26.86%). Education was primary level (75.50%) and income lower than 2,000 baht per month (68.50%). One year after discharged; patients had improved their function of extremities. In self-performance in daily life activities, most of patients had improvement in sitting (66.17%), eating (77.44%), tooth brushing and face washing (59.02%), dressing (31.58%), bathing and washing by assistant (47.70%), walking (63.80%), walking upstairs (51.70%), visiting neighbor (63.80%), participating social activities (36.10%). However 55.60% of them were incapable to do household works and 72.89% were unemployed. Related factors to mortality, from 32 patients died during the study, male had risk of death 2.43 time over than those of female (Odd Ratio = 2.43, 95% Cl = 1.176-5.198). The most potential cause of death was concomitance of diabetes, hypertension and heart disease which risk of death was 8.185 time higher than baseline (Odd Ration = 8.185, 95% Cl = 4.825-12.367).
Conclusion: One year after discharged from the hospital; most of patients had improved their function and activity. Nevertheless they were incapable to perform household work and occupation. Risk factors of these patients were male and with presence of diabetes, hypertension and heart disease which mortality was higher than those who had none or less concomitant diseases.
