Abstract

Background: Cerebral venous sinus thrombosis is an uncommon condition. The clinical presentation is varied. It usually affects young-to-middle-aged patients and more commonly women. And without immediate intervention, severe complications may cause of death.
Objective: To study clinical diagnosis, treatment and outcome including complications of cerebral venous sinus thrombosis patients at Surin hospital.
Setting: Surin hospital
Study design: Descriptive retrospective study
Method: Review of patients (aged above 15 years) data admitted with cerebral venous sinus thrombosis during 1 October 2004 to 30 September 2009.
Result: The study revealed the data of 8 patients (4 female, 4 male), aged between 19-41 years. The clinical manifestations were headache 8 patients, diplopia 2 patients, seizure 4 patients, hemiparesis 3 patients, spastic dyarthria and dysphagia 1 patient. Magnetic resonance image of the brain showed involvement of the superior sagittal sinus 6 patients, transverse sinus 3 patients, Sigmoid sinus 2 patients, Vein of Galen 1 patient. The important predisposing factors include protein ร deficiency 3 patients, polycythemia vera 1 patient, oral contraceptive pill use 1 patient and antiphospholipid syndrome 1 patient. One patient had underlying acute lymphoblastic leukemia received L-asperagenase. No possible cause for cerebral venous sinus could be identified in 1 patients. Seven patients received therapeutic anticoagulant (Low molecular weight heparin) and warfarin for long term therapy. One patient with deep venous sinus thrombosis died in this study.
Conclusion: Cerebral venous sinus thrombosis may present to the physician in many clinical manifestations. Diagnosis can be confirmed by MRI of the brain in most cases. Early recognition of the condition and investigation of appropriate therapy probably reduces morbidity and mortality.
