Efficacy and reliability of Mass Diabetic Retinopathy Screening by a Non-mydriatic Camera in Surin Province

Abstract

Background: Indirect Ophthalmoscope is originally used for diabetic retinopathy screening in Surin hospital. However this method is not practical for all patients according to staff inadequacy and inaccessibility to the service. Therefore using a mobile non-mydriatic fundus digital camera will enhance patient's accessibility to diabetic retinopathy screening service in the community. This technique enables trained nurses to provide diabetic retinopathy screening service in community hospitals of Surin province. Thus the efficacy and reliability of the screening are depend on interpretation skill of the screeners.
Objective: To compare the interpretation result performed by trained screening nurses and ophthalmologist.
Research design: Prospective study
Method: Screening nurse's interpretation by using non-mydriatic fundus digital camera was collected from 1st October 2006 to 30th September 2007. The retina images taken from diabetic patients in all districts of Surin province were interpreted by screening nurses. The result of interpretations was then compared to those of ophthalmologist.
Result: Correlation of diabetic retinopathy and maculopathy interpretation between trained nurses and ophthalmologist was highly acceptable (Kappa coefficient = 0.752 and 0.724 respectively). But correlation of grading of severity was moderate to low level (Kappa coefficient = 0.562).
Conclusion: The efficacy and reliability of diabetic reinopathy screening using a mobile non-mydriatic fundus digital camera performed by trained nurses is highly acceptable. Screening nurses are qualified to provide this service to diabetic patient in community level.
