Prevalence and Factors Associated with Shoulder Pain in Hemiplegic Patients : A Study at the Department of Physical Therapy, Buriram Hospital

Abstract
Objectives: To study the prevalance and factors associated with Shoulder pain in hemiplegic patients at the department of Physical Therapy, Buriram Hospital.
Study design: Descriptive study.
Setting: Department of physical therapy, Buriram hospital, Buriram province.
Subject: Fourteen hemiplegic patients who were refered to the department of Physical Therapy for rehabilitation in July 2003. They had been followed - up once a month untill Jan 2004.
Method: All hemiplegic patients were assessed by a physical therapist on occurance of shoulder pain, sensory and mortor deficits, quality of movement, and activity of daily living. Then, they were given a rehabilitation program for 2 - 3 times during admission in the hospital. After being discharge from the hospital, they were visited by the physical therapist and given a handbook for caring patients with hemiplegia so that they could look after themselves at home. Monthly follow-ups, including reassessment and suggestions, had been done for six visits
Result: Five of 14 hemiplegic patients (36%) had shoulder pain during the first three months after stroke. However, at the sixth month, only two patients (14.3%) were found to have shoulder pain. It was that sensory and mortor deficits were factors associated with the occurance of shoulder pain.
Conclusion: Prevalence of shoulder pain in patients with hemiplegia was found to be high, sensory and mortor deficits after stroke were associated with the occurance of shoulder pain. Early rehabilitation with a handbook of patient care, and regularly home visit for follow-up were recommended.
