Factors Associated with Rupture Appendicitis in Rural Hospital

Abstract
Background: Appendicitis occurs most often between the ages of 5 and 45, with a mean age of 28. In worldwide incidence is approximately 233/per 100,000 people and postoperative complication intra-abdominal abscess, adhesion band, surgical site infection(SSI) and long of stay (LOS) was impair to quality of life in patient.
Objectives: The aim of this study factors associated with rupture appendicitis in rural hospital of Thailand.
Method: A retrospective study was conducted in Lamduan hospital from January 2020 to December 2022. Those patients diagnosed as rupture appendicitis and acute appendicitis were included and statistical analysis by Multilogistic regression.
Result: A total of 139 patients were included in this study and divided 33 % (46 patients) in rupture appendicitis and 66 % (93 patients) in acute appendicitis. The mean age was 53.43 ± 14.53 in rupture appendicitis group and 47.12 ± 17.07 in acute appendicitis group. In multivariate analysis, factors associated with rupture appendicitis were long duration of abdominal pain AOR = 1.05 (95% CI 1.01, 1.08), present of generalize abdominal pain AOR = 35.67 (95% CI 6.7, 189.82) and increase Polymorph nuclear leukocyte(PMN) AOR = 1.09 (95% CI 1.01, 1.19).
Conclusion: Factors found to be associated with rupture appendicitis were long duration of abdominal pain, present of generalize abdominal pain and higher PMN count.
