Effect of a Health Literacy Promotion Program on Medication Adherence Using the LINE Application among Older Adults with Uncontrolled Hypertension at Health Promotion Hospital Region 9, Nakhon Ratchasima

This research and development study aimed to examine the effects of a health literacy enhancement program on medication adherence using the LINE application among older adults with uncontrolled hypertension. The study was conducted in three phases. Phase 1 involved an assessment of the situation and factors influencing medication adherence. The findings indicated that disease knowledge, health literacy, and family support were significant factors. Some older adults reported forgetting to take their medication and lacking understanding of continuous medication use. Phase 2 focused on the development of the health literacy enhancement program based on the findings from Phase 1. The program was designed, pilot-tested in a small group, and refined accordingly. It consisted of medication reminder notifications via the LINE application, educational videos, and infographics on appropriate medication use. Phase 3 evaluated the effectiveness of the program using a quasi-experimental design. The experimental and comparison groups included 35 participants each. The experimental group received the program through the LINE application for 12 weeks, while the comparison group received standard hospital care. The results showed that the experimental group had significantly higher health literacy and medication adherence scores than the comparison group (p<0.001). The mean blood pressure level significantly decreased from 142.03/90.88 to 131.23/84.06 mmHg (p<0.001). In conclusion, the health literacy enhancement program significantly improved medication adherence and reduced blood pressure levels among older adults with hypertension. The program can be applied and expanded to support the management of other chronic diseases in healthcare service units and local administrative organizations.
