Comparison of Darn Repair and Lichtenstein Mesh Repair for Inguinal Hernia

Abstract
Background: The groin hernia surgery is one of the common operations at Mahasarakham- Hospital.Tension free hernioplasty is necessary for a good hernia repair. There are many methods of tension-free hernioplasty practiced amongst which are the Darn repair and Lichtenstein mesh repair.
Objectives: To compare the result of inguinal hernia repair with Darn repair and Lichtenstein mesh repair by in relation to operating time, postoperative pain, postoperative complications and length of hospital stay.
Methods: This prospective study was conducted at Department of Surgery Mahasarakham Hospital from January 2009 to December 2010 including 80 patients with unilateral primary inguinal hernia .Of the total number, 40 patient sunderwent Darn repair, and 40 patients were operated by Lichtenstein mesh repair. We evaluated and compared operating time, postoperative pain, postoperative complications and length of hospital stay. Those presenting with strangulated, obstructed, recurrence inguinal hernia and patients with underlying diseased were excluded from study.
Results: This study had 80 patients, the majority were males aged between 40 - 50 years with indirect inguinal hernia. After comparing average operatingtime, postoperative pain score at 24 hours and postoperative complications between Darn repair andLichtenstein mesh repair groups, the result showed thataverage operating time, postoperative pain score at 24 hours, postoperative complications and length of hospital stay in both groups did not differ significantly. Additionally, at 1 year follow-up, the recurrence rate was 1 case in each group.
Conclusion: The study found that darn repair for inguinal hernia repair did not differ from Lichtenstein mesh repair in terms of operating time, postoperative pain, postoperative complications and length of hospital stay; however, the advantage of the Darn repair lies in the fact that it does not require Mesh, so it is much more cost effective.
